Onderstaande genentest, Fluorescence In Situ Hybridization (FISH), wordt meer en meer gebruikt, ook bij longkanker. (Ook bij borstkanker voor vaststellen van Her2-Neu expressie wordt FISH gebruikt) 

Met de testresultaten uit een FISH test kan een veel gerichtere behandeling gegeven worden aan longkankerpatiënten. Recent verscheen nog een publicatie waarin deze test voor longkanker aanvullend op de EGFR test werd geanalyseerd en uitgevoerd bij totaal 99 patiënten.

Een nieuwe test zou het behandelen van longkanker beter kunnen richten en voorspellen. Aldus een persbericht. Let wel het gaat om een test die niets zegt over de werking en de behandelingen. Er wordt verwacht dat er meer patienten een respons zullen geven op een gerichtere behandeling, maar dat zegt nog niets over het uiteindelijke resultaat. Hier het originele persbericht.

Op 5 juli zal in de pers lounge van het Wereld Longkanker Congres 2011 Dr. Marileila Varella Garcia,
professor medische oncologie aan de Universiteit van Colorado, Denver School of Medicine,  een
presentatie geven over de nieuwste ontwikkelingen op het gebied van personalized medicine &
companion diagnostics bij longkanker.  Zij zal een overzicht geven van nieuwe biomarkers 
(moleculen, waarmee een celtype/gen bepaald kan worden)  voor niet kleincellige longkanker.  
Prof. Garcia presenteert een nieuwe test, waarmee een herschikking van het ALK gen op te sporen is
dat de groei van niet- kleincellige longkanker veroorzaakt.  Door de nieuwe test in te zetten kunnen
artsen heel nauwkeurig patiënten identificeren voor de behandeling met nieuwe anti-kanker
medicijnen. Zo kan zeer gericht de therapie worden ingezet en een veel grotere respons worden
behaald dan voorheen.
 
Direct na de persconferentie en 6-7 juli  is er de mogelijkheid tot individuele interviews met Dr.
Garcia en Susan Jewell, Ph.D., Associate Director, Scientific Affairs, Abbott Molecular over de ALK-gen
test en waarom dit essentieel is voor de ontwikkeling van nieuwe medicijnen. Ook zal zij ingaan op de
rol die de test speelt in het selecteren van de juiste patienten met niet kleincellige longkanker voor
behandeling. 
 
Abbott introduceert in Europa de nieuwe moleculaire test en waardebepalingen om abnormale gen
herschikkingen op te sporen in niet klein-cellige longkanker. De test gebruikt Fluorescence In Situ
Hybridization (FISH) technologie om de herschikkingen te ontdekken van de ALK genen op de 2p23
chromosomen. Deze ALK gen herschikkingen  worden verantwoordelijk geacht voor de groei van niet
kleincellige longkanker.
 
The Abbott ALK test is een paradigma verschuiving in de ontwikkeling van diagnostiek en therapiën
voor niet klein-cellige longkanker  alsmede de opkomende wetenschap van gepersonaliseerde
medicatie.

Speekseltest zou beginnende vormen van kanker waaronder alvleesklierkanker aan kunnen tonen. Zo melden Japanse en Amerikaanse onderzoekers. Voor alvleesklierkanker zou dit mooi zijn omdat dit vaak pas ontdekt wordt als het al te laat is en er niet meer geopereerd kan worden.

TOKYO (AFP) – Japanese and US universities have jointly developed a medical technique that can quickly detect various cancers using a simple saliva test, researchers said on Tuesday.

Japan's Keio University and University of California, Los Angeles, have developed the technology with which they detected high probabilities of pancreatic cancer breast cancer and oral cancer.

The researchers analysed saliva samples of 215 people, including cancer patients, and identified 54 substances whose presence can be used to detect the disease, Keio University said in a statement released Monday.

By further analysing the substances, the test detected 99 percent of pancreatic cancer cases, 95 percent of breast cancer and 80 percent of oral cancer cases among those taking part, it said.

The cancer test using the technology would take half a day at the longest, the researchers said.

The new technology can detect up to 500 different substances found in saliva at one time, said professor Tomoyoshi Soga at Keio's Institute for Advanced Biosciences.

He said the technology would make it especially easier to detect pancreatic and oral cancers.

Fluorescence In Situ Hybridization and Immunohistochemistry as Diagnostic Methods for ALK Positive Non-Small Cell Lung Cancer Patients

Abstract

Background

Anaplastic Lymphoma Kinase (ALK) positivity represents a novel molecular target in a subset of Non-Small Cell Lung Cancers (NSCLC). We explore Fluorescence in situ Hybridization (FISH) and Immunohistochemistry (IHC) as diagnostic methods for ALK positive patients and to describe its prevalence and outcomes in a population of NSCLC patients.

Methods

NSCLC patients previously screened for Epidermal Growth Factor Receptor (EGFR) at our institution were selected. ALK positive patients were identified by FISH and the value of IHC (D5F3) was explored.

Results

ninety-nine patients were identified. Median age was 61.5 years (range 35–83), all were caucasians, eighty percent were adenocarcinomas, fifty-one percent were male and thirty-eight percent were current smokers. Seven (7.1%) patients were ALK positive by FISH, thirteen (13.1%) were EGFR mutant, and 65 (65.6%) were negative/Wild Type (WT) for both ALK and EGFR. ALK positivity and EGFR mutations were mutually exclusive. ALK positive patients tend to be younger than EGFR mutated or wt patients. ALK positive patients were predominantly never smokers (71.4%) and adenocarcinoma (71.4%). ALK positive and EGFR mutant patients have a better outcome than negative/WT. All patients with ALK FISH negative tumours were negative for ALK IHC. Out of 6 patients positive for ALK FISH with more tissue available, 5 were positive for ALK IHC and 1 negative.

Conclusions

ALK positive patients represent 7.1% of a population of selected NSCLC. ALK positive patients have different clinical features and a better outcome than EGFR WT and ALK negative patients. IHC is a promising method for detecting ALK positive NSCLC patients.
