Gliasite brachytherapie, een vorm van "vloeibare" bestraling van hersentunmoren waarbij zoveel mogelijk gezond weefsel wordt gespaard lijkt een redelijk alternatief voor de klassieke bestraling en zelfs voor de GammaKnife. In een studie met 20 patienten verdeeld over 8 verschillende ziekenhuizen werd een mediane overleving bereikt van 11 maanden voor Glioblastoom Multiforme. Dit zou ca. 3 maanden langer zijn dan statistisch mocht worden verwacht van deze patienten. Belangrijkste voordeel is dat de toxiciteit veel minder was voor de patient. Hiermee komt de behandeling met hyperthermie en dendritische celtherapie zoals dr. Gorter dat in Keulen toepast met o.a. hersentumoren wel in een bijzonder positief daglicht te staan. Volgende maand (december 2009) zullen we hopelijk weer een videoverhaal publiceren van een man die met een inoperabele hersentumor zijn toevlucht zocht in het Medisch Centrum Keulen nadat hij in Nederland werd opgegeven en nu na 1 jaar behandelen klinisch kankervrij is. GliaSite brachytherapy boost as part of initial treatment of glioblastoma multiforme: a retrospective multi-institutional pilot study.

PURPOSE: To report on a retrospective analysis of the cumulative experience from eight institutions using the GliaSite Radiotherapy System as a brachytherapy boost in the initial management of glioblastoma multiforme.
METHODS AND MATERIALS: Eight institutions provided data on 20 patients with histologically proven glioblastoma multiforme with a median age of 59 years (range, 39-76) and median Karnofsky performance scale of 80 (range, 50-100). After maximal surgical debulking, patients were treated with GliaSite brachytherapy to a median dose of 50 Gy, followed by external beam radiotherapy to a median dose of 60 Gy (range, 46-60 Gy), for a cumulative dose escalation of 110 Gy (range, 84-130 Gy).
RESULTS: The average survival for this study population was 11.4 months (range, 4-29). When the patients' survival was compared with that of historical controls according to their Radiation Therapy Oncology Group recursive partitioning analysis class, the average survival was increased by 3 months (95% confidence interval, 0.23-4.9) corresponding to a 43% increase (p = 0.033). Three patients (14%) experienced Radiation Therapy Oncology Group Grade 3 central nervous system toxicity. Of the treatment failures, 50% were >2 cm from the edge of the balloon.
CONCLUSION: The results of this analysis have demonstrated that dose escalation (>100 Gy) with GliaSite is well tolerated and associated with minimal toxicity. Local control improved with the use of GliaSite brachytherapy. The putative survival advantage seen in this study needs to be interpreted with caution; nevertheless, the data provide sufficient justification to investigate the potential role of radiation dose escalation in conjunction with GliaSite in the initial treatment of glioblastoma multiforme.

Nieuwe experimentele vorm van 'vloeibare' bestraling in ballon bij patiënt met hersentumoren na operatie met succes uitgevoerd. Volgend op chirurgische verwijdering van een hersentumor wordt een met vloeibare bestraling gevulde ballon ingebracht in de holte waar de tumor zich bevond. 

New York, NY (22 maart 2004) -- Een patiënt met uitgezaaide hersentumor is de eerste patiënt in het New York City gebied geworden om met het unieke GliaSite Radiation Therapy System (RTS) te worden behandeld. Een team van neurochirurgen en radiologen -- geleid door neurochirurg Dr. Susan Pannullo -- verrichte deze ingreep/behandeling aan het NewYork-Presbyterian Hospital/Weill Cornell Medical Center.


Volgend op de chirurgische verwijdering van de hersentumor van de patiënt, werd het GliaSite Radiation Therapy System (RTS) in de achterblijvende holte ingebracht. De vloeibare straling werd dan ingespoten en daarmee werd de ballon gevuld. De radioactieve vloeistof bleef in de ballon voor enkele dagen, waar de bestraling rechtstreeks naar het gebied in de hersenen werd gericht/gestuurd waar de kanker het meest waarschijnlijk terugkeren zou. 

"In tegenstelling tot verschillende conventionele bestralingen die tot cognitieve defecten zou kunnen leiden, behandelt de Gliasite brachytherapie enkel een bepaald gericht gebied in de hersenen," zegt Dr. Theodore Schwartz, de neurochirurg die de operatie van de tumor en plaatsing van de ballon verrichtte. Dr. Schwartz is assistent toeziend neurologisch chirurg aan het NewYork-Presbyterian Hospital/Weill Cornell Medical Center en assistent Professor van de neurologische chirurgie aan het Weill Cornell Medical College. 

Enkele dagen na de chirurgische verwijdering van de hersentumor en de implantatie van de Gliasite ballon, is begonnen met de brachytherapie behandeling. Een vaste dosis van straling wordt door de Iotrex uitgestraald -- een vloeibare bestralingsbron ontworpen om bepaalde patiënten met kwaadaardige tumors te behandelen -- nadat het in de ingebrachte ballon wordt ingespoten. De Iotrex bezorgt bestraling naar de randen van de tumorholte en wordt gericht op weefsel waar kanker zou kunnen zijn achtergebleven. De Iotrex blijft in de ballon tot de voorgeschreven hoeveelheid van straling toegediend is, nadien wordt het apparaat dat de Iotrex regelt verwijderd. 

Dr. David Sherr -- assistent toeziend radioloog en Residency Program Director aan het Stich Radiation Center at NewYork-Presbyterian/Weill Cornell en assistent professor van Clinical Radiology aan het Weill Cornell Medical College -- diende de Iotrex injectie toe.

Er is sprake van uitgezaaide hersenkanker wanneer kankercellen naar de hersenen van een patiënt met een primaire tumor elders in het lichaam uitzaaien en daar een tumor of meerdere tumoren vormen. Meer dan 170,000 Amerikanen worden ieder jaar met uitgezaaide hersentumoren gediagnosticeerd. Met traditionele therapieën is de gemiddelde overleving 11 maanden. Gliasite RTS is ook doeltreffend voor andere soorten primaire hersenkanker, zoals belangrijke hersentumoren genaamd Glioblastoma Multiforme. 

GliaSite Radiation Therapy System (RTS) is goedgekeurd door de FDA en wordt door Proxima Therapeutisch, Inc. Alpharetta, Georgië vervaardigd. 

NewYork-Presbyterian/Weill Cornell's Neuro-Oncology Division biedt het volledige spectrum aan van traditionele en nieuwe en vernieuwende behandelingen -- inclusief chirurgie, bestralingstherapiën, stereotactische radiologisch operaties, chemotherapie en complementaire therapieën. En brachytherapy, wat inhoudt dat de bron van bestraling rechtstreeks in de tumor wordt geplaats, heeft al lang de speciale interesse van het Radiation Oncology programma aan het NewYork-Presbyterian/Weill Cornell en wordt ook gebruikt om andere vormen van kanker te behandelen.

A New York City First: Brain Cancer Patient Receives Unique Liquid Radiation Treatment at NewYork-Presbyterian/Weill Cornell 

Following Surgical Removal of Brain Tumor, GliaSite Balloon Catheter Is Inserted in Brain Tumor Cavity, Where It Delivers Liquid Radiation

New York, NY (March 22, 2004) -- A patient with metastatic brain cancer has become the first in the New York City area to receive the unique GliaSite Radiation Therapy System (RTS). A team of neurosurgeons and radiation specialists -- led by neurosurgeon Dr. Susan Pannullo -- performed this procedure at NewYork-Presbyterian Hospital/Weill Cornell Medical Center.

Following the surgical removal of the patient's brain tumor, the GliaSite balloon catheter system was inserted into the remaining cavity. Liquid radiation was then injected to fill the balloon. The radioactive liquid remained in the balloon for several days, where it delivered radiation directly to the area in the brain where cancer would most likely recur.

"Unlike conventional radiation that may lead to cognitive defects, GliaSite brachytherapy treats only the targeted area in the brain," says Dr. Theodore Schwartz, the neurosurgeon who performed the removal of the tumor and placement of the balloon. Dr. Schwartz is Assistant Attending Neurological Surgeon at NewYork-Presbyterian Hospital/Weill Cornell Medical Center and Assistant Professor of Neurological Surgery at Weill Cornell Medical College.

Several days after the surgical removal of the brain tumor and the implantation of the GliaSite balloon catheter, the brachytherapy treatment is initiated. A steady dose of radiation is emitted by Iotrex -- a liquid radiation source specifically designed to treat patients with malignant tumors -- after it is injected into the implanted balloon. The Iotrex delivers radiation to the edges of the tumor cavity, targeting tissue where cancer may remain. The Iotrex remains in the balloon until the prescribed amount of radiation is delivered, after which time the device and the Iotrex are removed.

Dr. David Sherr -- Assistant Attending Radiologist and Residency Program Director at the Stich Radiation Center at NewYork-Presbyterian/Weill Cornell and Assistant Professor of Clinical Radiology at Weill Cornell Medical College -- administered the Iotrex injection.

Metastatic brain cancer occurs when cancer cells migrate to the brain from a primary cancer elsewhere in the body, and form a tumor or tumors there. More than 170,000 Americans are diagnosed every year with metastatic brain tumors. With traditional therapies, median survival is only 11 months. GliaSite RTS is also effective for other types of brain cancer, such as primary brain tumors like Glioblastoma Multiforme.

The GliaSite Radiation Therapy System (RTS), FDA-approved, is manufactured by Proxima Therapeutics, Inc., of Alpharetta, Georgia.

NewYork-Presbyterian/Weill Cornell's Neuro-Oncology Division offers the full spectrum of traditional and new and innovative treatments -- including surgery, radiation therapy, stereotactic radiosurgery, chemotherapy, and complementary therapies. Brachytherapy, which involves placing the source of radiation directly within the tumor, has long been a special interest of the Radiation Oncology program at NewYork-Presbyterian/Weill Cornell and is used to treat a range of cancers.
