In de bijlage Diagnose van het Algemeen Dagblad staat een o.i. heel goed informatief artikel over het nut en de voordelen van kijkoperaties bij darmkanker. Ook adressen van de Nederlandse ziekenhuizen waar deze kijkoperaties worden uitgevoerd. Een Spaanse studie van Dr. Lacy zou hebben uitgewezen - zie hieronder abstract van studie - dat het verwijderen van tumoren in de dikke darm d.m.v. de kijkoperatietechniek de kans op een recidief met misschien wel de helft teruggebracht kan worden. De Rotterdamse hoogleraar endoscopische chirurgie prof. dr. Jaap Bonjer relativeert die conclusies omdat hij vindt dat er nog meer studie naar gedaan moet worden , maar ook hij pleit voor meer kijkoperatieve operaties omdat z.i. de effectiviteit gelijk of beter is dan bij een open grote operatie en de patiënt onnodig lijden wordt bespaard. Er is veel minder bloedverlies, de patiënt sterkt veel sneller aan en hoeft ook veel minder lang in het ziekenhuis te verblijven. 

In ieder geval is in de Erasmus Rotterdam een gerandomiseerde studie - 1200 patiënten, duur drie tot zeven jaar - onder leiding van prof. dr. Bonjer opgezet. Hieronder, na de studiepublicatie van Dr. A. Lacy, het protocol zoals dat is gepubliceerd door de Erasmus. Eerst het abstract van de studieresultaten - gepubliceerd 28 juni 2002 in The Lancet - van dr. Antonio Lacy die in het artikel in het AD wordt genoemd als de chirurg die baanbrekend werk heeft verricht op dit gebied en ook door deze onderstaande gerandomiseerde studie onder 219 patiënten tot zijn opmerkelijke conclusies kwam. Dr. Lacy voert alleen in de hoognodige gevallen nog 'gewone' open operaties uit, maar het overgrote deel van de darmoperaties doen zij in het ziekenhuis in Barcelona waar hij werkt met de kijkoperatietechniek.

Summary 

Background Although early reports on laparoscopy-assisted colectomy (LAC) in patients with colon cancer suggested that it reduces perioperative morbidity, its influence on long-term results is unknown. Our study aimed to compare efficacy of LAC and open colectomy (OC) for treatment of non-metastatic colon cancer in terms of tumour recurrence and survival. 

Methods From November, 1993, to July, 1998, all patients with adenocarcinoma of the colon were assessed for entry in this randomised trial. Adjuvant therapy and postoperative follow-up were the same in both groups. The main endpoint was cancer-related survival. Data were analysed according to the intention-to-treat principle. 

Findings 219 patients took part in the study (111 LAC group, 108 OC group). Patients in the LAC group recovered faster than those in the OC group, with shorter peristalsis-detection (p=0·001) and oral-intake times (p=0·001), and shorter hospital stays (p=0·005). Morbidity was lower in the LAC group (p=0·001), although LAC did not influence perioperative mortality. Probability of cancer-related survival was higher in the LAC group (p=0·02). The Cox model showed that LAC was independently associated with reduced risk of tumour relapse (hazard ratio 0·39, 95% CI 0·19-0·82), death from any cause (0·48, 0·23-1·01), and death from a cancer-related cause (0·38, 0·16-0·91) compared with OC. This superiority of LAC was due to differences in patients with stage III tumours (p=0·04, p=0·02, and p=0·006, respectively). 

Interpretation LAC is more effective than OC for treatment of colon cancer in terms of morbidity, hospital stay, tumour recurrence, and cancer-related survival.

SUMMARY COLOR PROTOCOL

lntroduction & background

Laparoscopic resection of colorectal malignancies is controversial. Initial reports on port-site metastases have caused major concern. Although retrospective studies with large numbers of patients now suggest that the incidence of port-site metastases is comparable to the incidence of wound metastases in open surgery, the pathogenesis of these recurrences remains unclear.

Postoperative benefits of laparoscopic cholecystectomy are also seen after laparoscopic colon surgery and experimental work with animais indicaties that laparoscopic surgery might even result in lower recurrence rates. The only way to assess the value of laparoscopic surgery as therapeutic modality for colon cancer is a randomised trial.

Study design

The COLOR trial is a randomised, international, multi centre study comparing the outcomes of iaparoscopic and conventionai resection of colon carcinoma. Clinical and operative data will be collected centrally in the co-ordinating centre in Rotterdam, The Netherlands. Quality of life and costs will be assessed on a national basis.

Endpoints

Primary endpoint is cancer free survival at three years. Secondary endpoints are overall survival at three and five years, 28 day mortality and morbidity, quality of life, costs, location and rates of recurrences and pathologie anatomic characteristics of the resected specimen.

Statistics & randomisation

Using log rank statistics with a power of 80 % and an a of 0.05, 1200 patients are needed to detect a difference between both treatment arms of 7 % in 3 year cancer free survival. Analyses will be on `intention to treat basis`. The randomisation is stratified for type of resection (right hemicolectomy, left hemicolectomy or sigmoidectomy) and participating centre.

Main selection criteria

Patients with a single tumour in the caecum, ascending colon, descending colon aboral of the splenic flexure or sigmoid colon orally to the peritoneal deflection can be included. Not eligible are patients with concomitant metastases or other malignancies, with malignancies in their medical history, with signs of acute obstruction or with previous ipsilateral colon surgery.

Follow-up

Patients have to be seen at least once a year for five years. At visits 1, 2, 4 and 5 years after surgery, anamnesis and physicai examination are sufficient. Three years after surgery, colonoscopy or barium enema should be performed and liver and lungs have to be examined checked for metastases. In case of recurrent disease, follow up should be until 3 years from the time of diagnosis of recurrence.

d.d. 9/3/2003: in de bijlage Gezond van de Volkskrant stond gisteren (8 maart 2003) een aardig artikeltje over kijkoperaties enz. Op deze website van de NVEC is veel te lezen over kijkoperaties, het nut ervan en waar in Nederland er gebruik van wordt gemaakt. 

ROTTERDAM - Een groot deel van de medische instrumenten die bij kijkoperaties worden gebruikt, deugt niet. Dit blijkt uit een onderzoek van de Nederlandse Vereniging voor Endoscopische Chirurgie (NVEC) bij achttien Nederlandse ziekenhuizen. In totaal zullen 59 ziekenhuizen worden onderzocht. 

Volgens de onderzoekers kunnen door de defecten medische complicaties ontstaan. Bij 20 procent van de gebruikte instrumenten is volgens het onderzoek de isolatie beschadigd. Hierdoor zou de patiënt inwendige brandwonden kunnen oplopen. 

Eenderde van de lichtkabels die bij de kijkoperaties worden gebruikt, voldeed evenmin aan de normen. De lichtkabels brengen licht naar het te onderzoeken deel van het lichaam. 

Hoe meer licht, des te beter de arts op een monitor kan zien hoe ernstig de situatie is. "Als er weinig licht is", zegt voorzitter J. Bonjer van de NVEC, "kan de arts dingen over het hoofd zien." 

Bonjer zegt niet te weten of patiënten in het verleden complicaties hebben opgelopen door de ondeugdelijke apparatuur. De NVEC vindt dat alle ziekenhuizen hun instrumenten voor de kijkoperaties goed moeten onderzoeken en zo nodig moeten vervangen.

In Nederland worden per jaar 50.000 kijkoperaties uitgevoerd: 20.000 in de buik en 30.000 in de gewrichten. Volgens Bonjer is het een nieuwe methode die sinds tien jaar in Nederland op grote schaal wordt toegepast. Dit verklaart volgens hem waarom de organisatie nog niet op orde is. “Veel ziekenhuizen hebben tien jaar geleden nieuwe apparatuur aangeschaft. Die apparatuur is nu dringend aan vervanging toe. Dat is alleen nog niet doorgedrongen.”
