Probiotica - melkzuurbacteriën verminderen significant diarree veroorzaakt door bestraling bij patiënten met baarmoederhalskanker die daarvoor bestraald werden samen met een chemokuur - cisplatin. Ook was in de probioticagroep veel minder medicatie nodig om diarree tegen te gaan dan in de controlegroep. Dit blijkt uit de resultaten van een gerandomiseerde fase II studie bij totaal 63 patiënten.  De gebruikte probiotica waren levende lactobacillus acidophilus plus bifidobacterium bifidum. Probiotica kunt u kopen in bv. elke natuurwinkel, maar vaak ook in reformwinkels. Ook orthomoleculaire artsen schrijven vaak probiotica voor op recept. Hier een vertaling van het abstract. Achtergrond:
Door bestraling veroorzaakte diarree wordt vaak waargenomen tijdens radiotherapie van het bekken. Deze studie werd uitgevoerd om het effect van een probioticum van levende lactobacillus acidophilus plus bifidobacterium bifidum te testen op het voorkomen van diarree veroorzaakt door bestraling bij patiënten met lokaal gevorderde baarmoederhalskanker.

Methode:
Patiënten die werden bestraald op het bekken gelijktijdig tijdens een wekelijks chemokuur met cisplatine werden gerandomiseerd ingedeeld in een groep die alleen medicijnen kregen tegen diarree of een placebo in een dubbelblind onderzoek. Het optreden van diarree werd wekelijks vastgesteld volgens het Common Toxicity Criteria (CTC) systeem.
De vastheid van de ontlasting en witte en rode bloedcellen in de ontlasting werden ook beoordeeld. Het primaire eindpunt was de incidentie van diarree, gedefinieerd door een CTC graad 2 of meer en de behoefte aan anti diarree medicatie te verminderen.

Resultaten:
In totaal namen 63 patiënten deel aan de studie. Diarree met graad 2 -3 werd waargenomen bij 45% van de placebogroep (n = 31) en slechts bij 9% van de probioticagroep (n = 32) (p = 0,002).
Anti diarree medicatie gebruik was ook significant minder in de placebogroep (p = 0,03). De patiënten in de probioticagroep hadden ook een significant verbeterde vastheid van de
ontlasting (p <0,001).

Conclusies:
Probiotica met levende Lactobacillus acidophilus plus Bifidobacterium bifidum gaf een daling van de incidentie van bestraling geïnduceerde diarree en de behoefte aan anti-diarree medicatie en
had een significant positief effect op de vastheid van de ontlasting.

Background
Radiation-induced diarrhea is frequently observed during pelvic radiotherapy. This study was performed to determine the ability of a probiotic containing live lactobacillus acidophilus plus bifidobacterium bifidum to reduce the incidence of radiation-induced diarrhea in locally advanced cervical cancer patients.
Methods
Patients who were undergoing pelvic radiotherapy concurrent with weekly cisplatin were randomly assigned to a study drug or placebo, in a double-blind study. Diarrhea was graded weekly according the Common Toxicity Criteria (CTC) system. Stool consistency and white and red blood cell count in stool were also assessed. The primary endpoint was to reduce the incidence of diarrhea, defined by a CTC grade 2 or more, and the need for anti-diarrheal medication.
Results
A total of 63 patients were enrolled. Grade 2 -3 diarrhea was observed in 45% of the placebo group (n = 31) and 9% of the study drug group (n = 32) (p = 0.002). Anti-diarrheal medication use was significantly reduced in the placebo group (p = 0.03). The patients in the study drug group had a significantly improved stool consistency (p < 0.001).
Conclusions
Live lactobacillus acidophilus plus bifidobacterium bifidum reduced the incidence of radiation-induced diarrhea and the need for anti-diarrheal medication and had a significant benefits on stool consistency.
