Hoewel onderstaande berichtgeving uit  2005 van het LUMC suggereert dat bepaalde vormen van EPO ter bestrijding van bloedarmoede ontstaan door chemo een positief effect zou hebben, ook op de levensduur blijkt uit een grote overzichtstudie van 53 studies met in totaal 13.933 patienten dat EPO-verwante middelen wel de bloedarmoede en daarmee de kwaliteit van leven verhogen maar de levensduur verkleinen voor kankerpatienten.

Erythropoiesis-stimulerende middelen vergroot de kans op overlijden tijdens de actieve studieperiode (gecombineerd hazard ratio 1.17, 95% CI 1.06-1.30) en verslechtert de overall overlevingstijd (1.06, 1.00-1.12), met niet altijd 100% heterogeniteit tussen de studies (I (2) 0 %, p = 0,87 voor de sterfte tijdens de actieve periode van studie, en (2) 7,1%, p = 0,33 voor de totale overleving).

Toch suggereren de auteurs dat het welliicht toch goed is om EPO te gebruiken in verhogen van kwaliteit van leven. Het is wel een dilemma zoals ook enkele wetenschappers opmerken. Hier het nieuwste abstract uit de The Lancet van afgelopen maand:

BACKGROUND: Erythropoiesis-stimulating agents reduce anaemia in patients with cancer and could improve their quality of life, but these drugs might increase mortality. We therefore did a meta-analysis of randomised controlled trials in which these drugs plus red blood cell transfusions were compared with transfusion alone for prophylaxis or treatment of anaemia in patients with cancer.
 
METHODS: Data for patients treated with epoetin alfa, epoetin beta, or darbepoetin alfa were obtained and analysed by independent statisticians using fixed-effects and random-effects meta-analysis. Analyses were by intention to treat. Primary endpoints were mortality during the active study period and overall survival during the longest available follow-up, irrespective of anticancer treatment, and in patients given chemotherapy. Tests for interactions were used to identify differences in effects of erythropoiesis-stimulating agents on mortality across prespecified subgroups. FINDINGS: Data from a total of 13 933 patients with cancer in 53 trials were analysed. 1530 patients died during the active study period and 4993 overall. Erythropoiesis-stimulating agents increased mortality during the active study period (combined hazard ratio 1.17, 95% CI 1.06-1.30) and worsened overall survival (1.06, 1.00-1.12), with little heterogeneity between trials (I(2) 0%, p=0.87 for mortality during the active study period, and I(2) 7.1%, p=0.33 for overall survival). 10 441 patients on chemotherapy were enrolled in 38 trials. The cHR for mortality during the active study period was 1.10 (0.98-1.24), and 1.04 (0.97-1.11) for overall survival. There was little evidence for a difference between trials of patients given different anticancer treatments (p for interaction=0.42).
 
INTERPRETATION: Treatment with erythropoiesis-stimulating agents in patients with cancer increased mortality during active study periods and worsened overall survival. The increased risk of death associated with treatment with these drugs should be balanced against their benefits. Door een vraag in ons forum werden we geattendeerd op deze studie, waarvoor dank en hebben we enkele vervolgstudies met zelfde aanpak erbij gevoegd.

Verschillende mensen, waaronder een arts en waarvoor dank, wezen me erop dat EPO - als aanvulling bij bestraling bij mond- en keelkanker voor juist negatief effect zorgde, aldus fase III gerandomiseerde studie, zie ook in deze artikelenreeks. EPO maakt namelijk nieuwe bloedvaten aan en dat kan voor tumorgroei zorgen. Er wordt hierover verschillend gedacht in de oncologische wereld maar feit blijft wel dat onderstaande studies toch een opmerkelijk significant positief resultaat laat zien van Epoetine alfa - EPO naast chemokuren.

Het persbericht van het LUMC hebben we niet veel aan toe te voegen en lees ook abstract van andere studies met deze aanvullende aanpak van Epoetine alfa bij ander chemokuren als cisplatinum met zelfde uitstekende resultaten. 31% minder sterfgevallen dan in placebo controlegroep bleek uit fase III studie. Lees ook studie met heparine - zie onder onderzoek en voeding - die zelfde positieve effect bleek te hebben dan Epoetine alfa toonde het resultaat uit deelstudie.


Geneesmiddel tegen bloedarmoede verbetert kwaliteit van leven voor kankerpatiënten met chemotherapie

Kankerpatiënten die chemotherapie ondergaan, moeten tegelijkertijd een behandeling krijgen tegen bloedarmoede met het geneesmiddel epoetine alfa. Hierdoor verbetert de kwaliteit van leven voor deze patiënten aanzienlijk. Bovendien zijn er concrete aanwijzingen dat deze behandeling de levensverwachting verlengt.

Dit blijkt uit een recent artikel dat gepubliceerd is in het juninummer van het Amerikaanse tijdschrift "Journal of Clinical Oncology". Het onderzoek is mede uitgevoerd in het Diakonessen ziekenhuis in Utrecht, onder leiding van oncoloog Nortier, sinds kort hoogleraar in het Leids Universitair Medisch Centrum (LUMC).
Het onderzoek betreft een internationale gerandomiseerde studie onder 349 patiënten die of het geneesmiddel epoetine alfa kregen toegediend, of een placebo. De onderzoekers constateren dat patiënten met epoetine alfa significant meer energie hebben. Ze kunnen ook beter hun dagelijkse activiteiten uitvoeren dan patiënten die placebo krijgen. Bovendien hebben patiënten die dit geneesmiddel gebruiken, minder bloedtransfusies nodig dan patiënten in de placebogroep (28, resp. 40%).

Prof. Nortier: "Het is een zeer waardevol geneesmiddel. Het verbetert aantoonbaar de kwaliteit van leven voor kankerpatiënten. Eigenlijk zouden steeds meer patienten hierover moeten kunnen beschikken". Per jaar krijgen namelijk 60.000 mensen in Nederland te horen dat ze kanker hebben. Ongeveer een kwart daarvan krijgt een behandeling met chemotherapie.

Uit analyse van de onderzoekers blijkt dat patiënten die epoetine alfa gebruiken, een betere levensverwachting hebben, vergeleken met patiënten die een placebo ontvangen. Het overlijdensrisico in de placebo groep is ongeveer 31% hoger dan bij de patiënten die epoetine alfa krijgen.

Prof. Nortier: "Voor het eerst is bij patiënten die chemotherapie krijgen de trend waargenomen dat behandelen van bloedarmoede met epoetine alfa resulteert in een betere overleving. Een definitieve conclusie kunnen we nog niet trekken, omdat dit niet de primaire vraagstelling voor dit onderzoek was - die ging over de kwaliteit van leven. Wel leert het ons dat we patiënten met kanker eerder dan we vroeger deden moeten behandelen voor bloedarmoede.

Epoetine alfa wordt via recombinant DNA technieken gemaakt en is gelijk aan het natuurlijke hormoon erytropoetine. Het stimuleert de aanmaak van rode bloedcellen in het beenmerg. Het wordt gebruikt bij patiënten met chronisch nierziekten, bij patiënten met kanker die chemotherapie ondergaan en bij orthopedische operaties.

Effectiveness and safety of an induction therapy with epoetin alfa in anemic cancer patients receiving concomitant chemotherapy.

BACKGROUND: Epoetin alfa, administered at standard dosages of 10,000-20,000 IU three times weekly or 40,000-60,000 IU once weekly, has been shown to significantly increase hemoglobin (Hb) levels, decrease transfusion requirements, and improve quality-of-life parameters in patients undergoing chemotherapy.Objective. This open-label, nonrandomized, historically controlled study was conducted to evaluate the efficacy and safety of an induction dose of epoetin alfa in patients with moderate or severe anemia who were receiving chemotherapy.

METHODS: Nineteen patients with solid tumors and Hb levels < 9.0 g/dl were enrolled. The patients received single s.c. injections of epoetin alfa, 40,000 IU, on study days 1, 4, 7, 10, and 13, and were then observed for the following 30 days. Nineteen other cancer patients who had matching characteristics and had received epoetin alfa, 10,000 IU, three times weekly for the 45-day study period, served as historical controls. The primary efficacy variable was change in Hb level from baseline to days 15 (approximately week 2) and 45 (approximately week 6.5). Secondary efficacy variables included the percent response (Hb increase > or = 1 g/dl) and percent major response (Hb increase > or = 2 g/dl) at days 15 and 45, the durations of response and major response after day 45, the proportion of patients transfused within the 45 study days, the changes in Eastern Cooperative Oncology Group performance status score at days 15 and 45, and the ability to maintain the planned chemotherapy dose (dose intensity) over the 45-day study.

RESULTS: Mean increases in Hb level in the epoetin alfa 40,000 IU group were significantly greater than those in the historical control group both at day 15 and at day 45. The increase in Hb level in the control group approximated increases reported with standard 3-times-weekly epoetin alfa at day 15 but was somewhat lower than the increases typically seen by day 45, presumably due to the fact that, in the present study, the epoetin alfa dose was not doubled in initial nonresponders, as is commonly done with standard epoetin alfa treatment. The rates of major response for epoetin alfa 40,000 IU patients (37% at day 15 and 84% at day 45) were higher than those for control patients (16% and 21%, respectively). Also, the transfusion rate was lower and performance status scores were better in the epoetin alfa 40,000 IU patients than in the control patients. In all, 74% of epoetin alfa 40,000 IU patients versus 47% of control patients received 100% of the planned chemotherapy dose. Epoetin alfa was well tolerated in both treatment groups.

CONCLUSIONS: Results of this study suggest that epoetin alfa at a dose of 40,000 IU administered five times over 2 weeks may confer even higher response rates than those seen with standard dosing regimens. These encouraging results support further study of the proposed induction dose of epoetin alfa in a larger, randomized, prospectively controlled trial. Copyright AlphaMed Press

BACKGROUND: Chronic anemia is a well-recognized complication of both cancer and cytotoxic treatments and is associated with symptoms (e.g., fatigue, dyspnea) that may induce or exacerbate functional deterioration. The use of recombinant human erythropoetin (rHuEPO epoetin alfa) clearly increased haemoglobin (Hb) levels, decreased transfusion needs and allowed recovery of quality of life in anemic cancer patients (pts) undergoing chemotherapy (CT). The purpose of this open-label, non randomized, pilot study was to assess the safety and efficacy of an intensive 19-day epoetin alfa treatment in anemic patients with solid tumors receiving chemotherapy.

PATIENTS AND METHODS: Treatment: patients received a single induction s.c. dose of epoetin alfa 40,000 IU day 1 and twice a dose of 10,000 IU s.c. (8.00 a.m.- 8:00 p.m.) on days 3, 5, 8, 10, 12, 15, 17 and 19. The total dose of epoetin alfa per patient was 200, 000 IU. Iron supplementation: 125 mg i.v. days and 8. Soluble transferrin receptor (sTfR) levels were performed on days 1,8 and 15. This epoetin induction regimen was not followed by an epoetin maintenance therapy. PATIENTS: Twenty-nine anemic (Hb< or =11.5 g/dL) pts with non myeloid malignancies undergoing CT were included in the study.

RESULTS: At baseline the mean Hb level was 9.41 g/dl. On day 8, the mean Hb level increased to 10.07 g/dl (p<0.0001), reaching 10.68 g/dl on day 15 (p<0.0001). On days 22 and 29, the mean Hb levels increased to 10.93 and 11.05 g/dl, (p=0.002 and 0.033, respectively). No patient received blood transfusions. The global mean increase of Hb level was 1.64 g/dl (basal to d 29). It was defined as a major response: an increase of Hb levels > 1.5 g/dl. A rate of 62% (18/29 patients) of major responses was observed on day 21. Moreover, 25/29 patients (86.2%) presented an increase of Hb levels > 1 g/dl after 21 days. On days 8 and 15, the mean sTfR levels had increased significantly ( p=0.021 and 0.001, respectively). The increase of mean sTfR level after 15 days correlated significantly with the increase of mean Hb level in the first two weeks of epoetin therapy (p=0.05). Epoetin alfa has been well tolerated so far in the study.

CONCLUSION: The results of the present study suggest that an induction dose of 40,000 IU of epoetin alfa, followed by 8 maintenance doses of 20,000 IU each, may improve the standard response in terms of both time to response and Hb increase. Moreover, the Hb levels seemed to increase after epoetin therapy discontinuation (d22-29).

PURPOSE: Epoetin alfa administered s.c. three times weekly or once weekly increases hemoglobin (Hb) levels, decreases transfusion requirements, and improves quality of life in anemic cancer patients receiving chemotherapy. This study assessed the feasibility of using higher initial doses of once-weekly epoetin alfa followed by less frequent maintenance doses to increase and then maintain adequate Hb levels in this population.

MATERIALS AND METHODS: In this open-label, nonrandomized, pilot study, anemic (baseline Hb < or = 11 g/dl) cancer patients undergoing chemotherapy received initial doses of epoetin alfa of 60,000 U s.c. once weekly to increase Hb levels by at least 2 g/dl, followed by 120,000 U s.c. every 3 weeks to maintain Hb levels. The maximum treatment duration was 24 weeks.

RESULTS: The mean baseline Hb level was 10.1 +/- 0.8 g/dl (n = 20). Once-weekly dosing resulted in mean Hb level increases of 1.0 +/- 1.1 g/dl by week 4 and 2.9 +/- 1.9 g/dl by week 8; 86% and 79% of patients evaluable at week 8 and week 12, respectively, demonstrated increases of at least 2 g/dl (target Hb level of > or = 12 g/dl). Thirteen patients (65%) received at least one maintenance dose; the mean Hb level increased from 12.8 +/- 1.1 g/dl before starting maintenance therapy to 13.3 +/- 1.4 g/dl at the last maintenance week. Both dosage regimens were well tolerated.

CONCLUSIONS: Once-weekly epoetin alfa at a dose of 60,000 U effectively increased Hb levels by week 8; 86% of patients achieved rises of at least 2 g/dl or Hb levels > or = 12 g/dl. Moreover, epoetin alfa at doses of 120,000 U every 3 weeks maintained or increased Hb levels. Results from this pilot study suggest that higher initial once-weekly dosing of epoetin alfa followed by less frequent maintenance dosing appears to be feasible for treating anemia in cancer patients undergoing chemotherapy. Further evaluation of these and other epoetin alfa dosage regimens is warranted.
