Wanneer prostaatkankerpatiënten die bij de diagnose een aantal hoog risico factoren hadden, (≥T2c, of PSA ≥20, of Gleason Score ≥8 ref..1) statines alleen of samen met metformin gingen gebruiken dan verminderde de kans om te overlijden specifiek aan prostaatkanker met 54 procent en de kans om te overlijden aan alle oorzaken verminderde met 32 procent. Alleen gebruik van metformin gaf geen echte verschillen te zien. Statinegebruik alleen wel.

Het grootste effect werd gezien bij die patiënten die pas na de diagnose statines en metformin gingen gebruiken. Patiënten die al statines en / of metformin gebruikten voor de diagnose hadden ook nog wel minder kans te overlijden maar de percentages waren minder. 

Dat blijkt uit een studie gepubliceerd online 8 februari in Cancer Medicine. Zij onderzochjten de gegevens van 12.700 patiënten met hoog-risico prostaatkanker. De onderzoekers onder leiding van Xiang-Lin Tan, M.D., Ph.D., from Rutgers, The State University of New Jersey gebruikten de gegevens van de database van de Surveillance, Epidemiology, and End Results (SEER)‐Medicare.

De onderzoekers schrijven dat van alle prostaatkankerpatiënten 15 procent bij de eerste diagnose in de categorie valt waarvan zij de gegevens hebben geanalyseerd. (Ref:.1) Patiënten met een hoog risico prostaatkannker bij de diagnose hebben een grote kans op het ontwikkelen van systemisch of lokaal recidief en lopen een hoger risico op overlijden door de ziekte. 

Daarom worden wereldwijd veel studies gedaan om de sterfte aan prostaatkanker te verminderen met behoud van kwaliteit van leven en richten de studies zich vooral op deze groep van prostaatkankerpatiënten met hoog risico factoren. De meerderheid van de prostaatkankerpatiënten die later een uitgezaaide ziekte ontwikkelen, hebben bij de diagnose vaak nog een gelokaliseerde ziekte met een hoog risico. (Ref.2)

Er zijn aanwijzingen dat twee veel voorgeschreven geneesmiddelen met vastgestelde veiligheidsprofielen, metformine en statines, veelbelovende anti-kanker effecten hebben en geassocieerd zijn met een lagere mortaliteit bij prostaatkankerpatiënten (ref..3, 4). 

Onlangs hebben preklinische gegevens aangetoond dat een combinatie van metformine en statine beter was dan beide geneesmiddelen alleen bij het remmen van primaire tumorgroei, uitzaaiingen naar bot en biochemisch falen; gegevens bij de mens over de gezamenlijke effecten van statines en metformine op de prostaatkanker mortaliteit zijn echter beperkt. Ref:10, 11. 

Omdat het merendeel van de gebruikers van metformine ook statines gebruikt (ref: 12, 13) kunnen sommige van de gunstige resultaten die worden waargenomen bij gebruikers van metformine evengoed veroorzaakt zijn door statinegebruik.

Abstract

Background

Pre‐clinical studies suggest that metformin and statins may delay prostate cancer (PCa) metastases; however, data in humans are limited. To the best of our knowledge, this is the first human study aimed to quantify the individual and joint effects of statin and metformin use among patients with high‐risk PCa.

Methods

This population‐based retrospective cohort study identified patients from the Surveillance, Epidemiology, and End Results (SEER)‐Medicare linked database. Exposure to metformin and statins was ascertained from Medicare Prescription Drug Event files. The association with all‐cause and PCa mortality were evaluated using Cox proportional hazard model with competing causes of death, where propensity scores were used to adjusted imbalances in covariates across groups.

Results

Based on 12 700 patients with high‐risk PCa, statin alone or in combination with metformin was significantly associated with reduced all‐cause mortality (Hazard Ratio : 0.89; 95% Confidence Interval : 0.83, 0.96; and HR: 0.75; 95% CI, 0.67‐0.83, respectively) and PCa mortality (HR, 0.80; 95% CI: 0.69, 0.92) and 0.64; 95% CI, d 0.51‐0.81, respectively. The effects were more pronounced in post‐diagnostic users: combination use of metformin/statins was associated with a 32% reduction in all‐cause mortality (95% CI, 0.57‐0.80), and 54% reduction in PCa mortality (95% CI, 0.30‐0.69). No significant association of metformin alone was observed with either all‐cause mortality or PCa mortality.

Conclusions

Statin use alone or in combination with metformin was associated with lower all‐cause and PCa mortality among high‐risk patients, particularly in post‐diagnostic settings; further studies are warranted.

    Van red yeast rice is in vitro ook een remmend effect gevonden tav prostaatkanker. In vivo zijn er in diermodellen bij diverse kankers ook van metformine (maar ook van berberine dat veel op metformine lijkt)antikankereffecten gevonden. Het punt blijft dat het onderzoek hier genoemd slechts een epidemiologisch onderzoek is en geen RCT.Naar een definitief bewijs moet nog gezocht worden!!
