Een immuuntherapeutische behandeling met rindopepimut (CDX-110, Celldex Therapeutics) plus Avastin - Bevacizumab verlengt ook de overall overleving en ziektevrije tijd bij patiënten met een recidief van een Glioblastoma. Bljikt uit de tussenresultaten van een fase II studie. Eerder al zoals hieronder beschreven verbeterde de progressie-vrije overleving en de mediane algehele overleving bij nieuw gediagnosticeerde patiënten met een hersentumor Glioblastoom multiforme.

Nu blijkt uit tussenresultaten van een fase II studie dat de progressievrije tijd met rindopepimut naast temodal en Avastin met 30 procent verbeterde ten opzichte van Temodal met alleen Avastin - bevacizumab.

De progressievrije tijd voor patiënten in de rindopepimut naast temodal en Avastin was 27% vergeleken met 11% voor patiënten behandeld in de controlegroep met alleen Avastin plus temodal. 

De mediane overall overleving bedraagt voor rindopepimut naast temodal + Avastin 12 maanden vergeleken met 8.8 maanden in de Avastin controlegroep. Een verschil van 3.2 maanden en is statistisch significant. Maar 1 jaar leven met een hersentumor van het type Glioblastoma is nagenoeg normaal te noemen en ik zie niet zo snel het echte voordeel. Ook de FDA heeft eerder al een aanvraag tot toelating afgewezen voor deze aanpak.

Bedenk hierbij ook dat in Europa Avastin niet meer wordt gegeven naast temodal bij hersentumoren omdat dat geen enkele zin zou hebben. Bovendien hebben andere vormen van immuuntherapie bewezen veel effectiever te zijn, zie o.a. in gerelateeerde artikelen. M.i. zijn andere vormen van immuuntherapie ver te prevaleren boven deze aanpak, maar ja.......Bovenstaande informatie heb ik uit een persbericht van Wallstreet gehaald: Celldex Immunotherapy Prolongs Survival for Recurrent Brain Tumor Patients en zegt veel over het belang van deze publ;icatie. In Pubmed is deze studie nog niet opgenomen.

Vorig jaar werd een studierapport van een fase III studie gepubliceerd: A phase II, multicenter trial of rindopepimut (CDX-110) in newly diagnosed glioblastoma: the ACT III study  dat gratis is in te zien.

Een experimenteel vaccin gericht op de epidermal growth-factor receptor variant III (EGFRvIII) verbetert de progressie-vrije overleving en de mediane algehele overleving bij nieuw gediagnosticeerde patiënten met een hersentumor Glioblastoom multiforme in een kleine fase II studie

Het vaccin, bekend als rindopepimut (CDX-110, Celldex Therapeutics), werd gebruikt bij 18 patiënten die ook behandeld waren met een volledige operatie, externe bestraling, en gelijktijdig Temozolomide - Temodal. De resultaten uit de vaccin groep werd vergeleken met een gematchte controlegroep van 17 patiënten die werden behandeld met dezelfde basis behandelingen maar zonder het vaccin rindopepimut.
Patiënten in het vaccin groep kregen 3 initiële intradermale vaccinaties per 2 weken te beginnen vanaf 4 weken na de voltooiing van de bestraling, en daarna elke maand tot en met het bewijs van progressie d.m.v. een scan. Alle patiënten in beide groepen hadden primaire hersenkanker Glioblastoma Multiforme met  EGFRvIII expressie.

Na correctie voor leeftijd en Karnofsky performance status, de totale overleving van de gevaccineerde patiënten was 5-keer beter dan die in de controlegroep (hazard ratio 5.3; P = 0,0013).
Totale overleving was het langst bij de gevaccineerde patiënten die als reactie op de vaccinatie specifieke antilichamen aanmaakten of vertraagde overgevoeligheid voor de EGFRvIII expressie.

Het vaccin vergrootte de mediane overlevingstijd van statistisch verwachte 15 maanden tot 26 maanden. Patiënten in het vaccin groep  ervaarden ook een veel langere progressie-vrije overleving periode dan degenen die niet het vaccin hadden gekergen (14,2 vs 6,3 maanden).

De onderzoekers ontdekten dat het peptide vaccin kankercellen die EGFRvIII gevoelig waren elimineerden. Bij hernieuwd onderzoek werd EGFRvIII expressie niet langer gevonden bij 82% van de gevaccineerde patiënten.

Het vaccin heeft geen enkele symptomatische auto-immuunreacties of bijwerkingen.

Onderzoeksleider John Sampson, MD, van de Duke University Medical Center in Durham, North Carolina, zei: "Onze studie resultaten tonen aan dat het vaccineren van nieuw gediagnosteerde patiënten met glioblastoma multiforme met EGFRvIII-positieve expressie en die behandeld worden met een peptide vaccin die een EGFRvIII-specifieke epitoop aanpakt, veilig is en indiceert specifieke immuniteit tegen EGFRvIII expressie en wordt geassocieerd met de eliminatie van EGFRvIII-expressie cellen na herhaling.
Deze observatie dat een EGFRvIII gerichte vaccin in staat is EGFRvIII-expressieve tumorcellen te eliminreren bij de meerderheid van de patiënten is een intrigerend gegeven, en is consistent met wat we vinden in onze preklinische studies bij muizen "

Dr Sampson merkt ook op dat een voordeel van dit vaccin is dat het beschikbaar is "off the shelf" en geen arbeidsintensieve celpreparaat technieken vereist. Een nadeel is dat slechts een
derde van glioblastoma multiforme patiënten  tumoren hebben met EGFRvIII expressie.

Immunologic Escape After Prolonged Progression-Free Survival With Epidermal Growth Factor ReceptorVariant III Peptide Vaccination in Patients With NewlyDiagnosed Glioblastoma.

Abstract

PURPOSE Immunologic targeting of tumor-specific gene mutations may allow precise eradication of neoplastic cells without toxicity. Epidermal growth factor receptor variant III (EGFRvIII) is a constitutively activated and immunogenic mutation not expressed in normal tissues but widely expressed in glioblastoma multiforme (GBM) and other neoplasms.

PATIENTS AND METHODS A phase II, multicenter trial was undertaken to assess the immunogenicity of an EGFRvIII-targeted peptide vaccine and to estimate the progression-free survival (PFS) and overall survival (OS) of vaccinated patients with newly diagnosed EGFRvIII-expressing GBM with minimal residual disease. Intradermal vaccinations were given until toxicity or tumor progression was observed. Sample size was calculated to differentiate between PFS rates of 20% and 40% 6 months after vaccination. Results There were no symptomatic autoimmune reactions. The 6-month PFS rate after vaccination was 67% (95% CI, 40% to 83%) and after diagnosis was 94% (95% CI, 67% to 99%; n = 18). The median OS was 26.0 months (95% CI, 21.0 to 47.7 months). After adjustment for age and Karnofsky performance status, the OS of vaccinated patients was greater than that observed in a control group matched for eligibility criteria, prognostic factors, and temozolomide treatment (hazard ratio, 5.3; P = .0013; n = 17). The development of specific antibody (P = .025) or delayed-type hypersensitivity (P = .03) responses to EGFRvIII had a significant effect on OS. At recurrence, 82% (95% CI, 48% to 97%) of patients had lost EGFRvIII expression (P < .001).

CONCLUSION EGFRvIII-targeted vaccination in patients with GBM warrants investigation in a phase III, randomized trial.

A phase II, multicenter trial of rindopepimut (CDX-110) in newly diagnosed glioblastoma: the ACT III study

Abstract

Background

The epidermal growth factor receptor variant III deletion mutation, EGFRvIII, is expressed in ∼30% of primary glioblastoma and linked to poor long-term survival. Rindopepimut consists of the unique EGFRvIII peptide sequence conjugated to keyhole limpet hemocyanin. In previous phase II trials (ACTIVATE/ACT II), rindopepimut was well tolerated with robust EGFRvIII-specific immune responses and promising progression-free and overall survival. This multicenter, single-arm phase II clinical trial (ACT III) was performed to confirm these results.

Methods

Rindopepimut and standard adjuvant temozolomide chemotherapy were administered to 65 patients with newly diagnosed EGFRvIII-expressing (EGFRvIII+) glioblastoma after gross total resection and chemoradiation.

Results

Progression-free survival at 5.5 months (∼8.5 mo from diagnosis) was 66%. Relative to study entry, median overall survival was 21.8 months, and 36-month overall survival was 26%. Extended rindopepimut vaccination (up to 3.5+ years) was well tolerated. Grades 1–2 injection site reactions were frequent. Anti-EGFRvIII antibody titers increased ≥4-fold in 85% of patients, and increased with duration of treatment. EGFRvIII was eliminated in 4/6 (67%) tumor samples obtained after >3 months of therapy.

Conclusions

This study confirms, in a multicenter setting, the preliminary results seen in previous phase II trials of rindopepimut. A pivotal, double-blind, randomized, phase III trial (“ACT IV”) is under way.
