Hoewel bij veel vormen van kanker immuuntherapie het beste resultaat geeft bij een hoge tumor last (mismatch repair deficiency) reageren hersentumoren - Glioblastomas multiforme weinig of niet op immuuntherapie met anti-PD medicijnen - checkpointremmers. Wel reageren hersentumoren goed op immuuntherapie met een gemoduleerd virus, zoals bv een gemoduleerd poliovirus.

Uit een zogeheten genomische analyse (DNA mutatieonderzoek) van tumorweefsel van patiënten met een hersentumor die deelnamen aan een onderzoek naar immuuntherapie met een gemoduleerd poliovirus (poliovirotherapie) en uit de gegevens van eerder gepubliceerde studies van patiënten met een hersentumor die werden behandeld met anti-PD medicijnen - checkpointremmers blijkt dat juist een hele lage tumormutatie belasting werd geassocieerd met een langere mediane overall overleving voor die patiënten.

Transcriptomische analyses lieten een omgekeerde relatie zien tussen tumormutatielast en verrijking van ontstekings gen-signaturen in studies bij patiënten van een recidief van glioblastommtumoren, maar niet bij nieuw gediagnosteerde patiënten met een glioblastoom. Dit suggereert volgens de onderzoekers dat er een relatie bestaat tussen tumormutatiebelasting en tumor-intrinsieke ontstekingen die zich ontwikkelden bij een recidief en mogelijk het gevolg kan zijn van de standaardbehandeling. Deze ontstekingsstaat van recidiverende glioblastomen bij aanvang bepaalde mogelijk hun gevoeligheid voor immunotherapie.

Deze studie werpt volgens de onderzoekers de mogelijkheid op dat een lage mutatiebelasting van de tumor kan worden gebruikt om de respons op immunotherapieën bij recidiverende glioblastomen te voorspellen. Maar uiteraard zal nog veel meer onderzoek nodig zijn.

Recurrent glioblastoma multiforme (GBM) is incurable with current therapies. Biallelic mismatch repair deficiency (bMMRD) is a highly penetrant childhood cancer syndrome often resulting in GBM characterized by a high mutational burden. Evidence suggests that high mutation and neoantigen loads are associated with response to immune checkpoint inhibition.

We performed exome sequencing and neoantigen prediction on 37 bMMRD cancers and compared them with childhood and adult brain neoplasms. Neoantigen prediction bMMRD GBM was compared with responsive adult cancers from multiple tissues. Two siblings with recurrent multifocal bMMRD GBM were treated with the immune checkpoint inhibitor nivolumab.

All malignant tumors (n = 32) were hypermutant. Although bMMRD brain tumors had the highest mutational load because of secondary polymerase mutations (mean, 17,740 ± standard deviation, 7,703), all other high-grade tumors were hypermutant (mean, 1,589 ± standard deviation, 1,043), similar to other cancers that responded favorably to immune checkpoint inhibitors. bMMRD GBM had a significantly higher mutational load than sporadic pediatric and adult gliomas and all other brain tumors (P < .001). bMMRD GBM harbored mean neoantigen loads seven to 16 times higher than those in immunoresponsive melanomas, lung cancers, or microsatellite-unstable GI cancers (P < .001). On the basis of these preclinical data, we treated two bMMRD siblings with recurrent multifocal GBM with the anti–programmed death-1 inhibitor nivolumab, which resulted in clinically significant responses and a profound radiologic response.

This report of initial and durable responses of recurrent GBM to immune checkpoint inhibition may have implications for GBM in general and other hypermutant cancers arising from primary (genetic predisposition) or secondary MMRD.


Mechanisms and therapeutic implications of hypermutation in gliomas

Abstract

A high tumour mutational burden (hypermutation) is observed in some gliomas1,2,3,4,5; however, the mechanisms by which hypermutation develops and whether it predicts the response to immunotherapy are poorly understood. Here we comprehensively analyse the molecular determinants of mutational burden and signatures in 10,294 gliomas. We delineate two main pathways to hypermutation: a de novo pathway associated with constitutional defects in DNA polymerase and mismatch repair (MMR) genes, and a more common post-treatment pathway, associated with acquired resistance driven by MMR defects in chemotherapy-sensitive gliomas that recur after treatment with the chemotherapy drug temozolomide. Experimentally, the mutational signature of post-treatment hypermutated gliomas was recapitulated by temozolomide-induced damage in cells with MMR deficiency. MMR-deficient gliomas were characterized by a lack of prominent T cell infiltrates, extensive intratumoral heterogeneity, poor patient survival and a low rate of response to PD-1 blockade. Moreover, although bulk analyses did not detect microsatellite instability in MMR-deficient gliomas, single-cell whole-genome sequencing analysis of post-treatment hypermutated glioma cells identified microsatellite mutations. These results show that chemotherapy can drive the acquisition of hypermutated populations without promoting a response to PD-1 blockade and supports the diagnostic use of mutational burden and signatures in cancer.
