Uit een prospectieve studie blijkt dat een Rezūm behandeling, een minimaal invasieve transurethrale waterdamptherapie voor de behandeling van lagere urinewegsymptomen gerelateerd aan goedaardige prostaathyperplasie - goedaardige prostaattumoren, effectief kan verbeteren. De algehele International Prostate Symptom Score (IPSS) en kwaliteit van leven verbeterden tot 1 jaar postoperatief. Bovendien werden vroege en aanhoudende verbeteringen waargenomen in de maximale urinestroom en het restvolume na de lediging van de blaas. 

In de studie werden 229 patiënten (gemiddelde leeftijd 67,3 jaar) opgenomen, waaronder 83 met prostaatvolumes ≥ 80 ml, en werden behandeld van april 2019 tot december 2020.
Het gemiddelde prostaatvolume was 71,5 ml (bereik 20-160 ml) en 55% had een mediane kwab.
Het gemiddelde aantal injecties was 11 (bereik: 4-28) en de gemiddelde procedureduur was 4,8 minuten (bereik: 1,5-14).
De gemiddelde duur van de katheterisatie na de procedure was 9,8 dagen. IPSS-scores verbeterden ten opzichte van baseline met respectievelijk 29%, 53% en 59% na 1, 3 en 12 maanden. IPSS QoL verbeterde ten opzichte van baseline met respectievelijk 30%, 50% en 67% na respectievelijk 1, 3 en 12 maanden.

De conclusie van de onderzoekers is dan ook positief:

Rezūm-therapie is een veilige, effectieve en snelle poliklinische procedure voor goedaardige prostaattumoren over een breed scala aan volumes. Klinisch significante verbeteringen worden gezien in alle gevalideerde vragenlijsten. Objectieve maximale stroommetingen verbeterden, terwijl de erectiele en ejaculatiefunctie behouden blijft.
Rezūm Waterdamptherapie is een niet-chirurgische behandeling die de natuurlijke energie gebruikt die is opgeslagen in waterdamp of stoom, om overtollig prostaatweefsel te verwijderen dat op de plasbuis drukt.

Rezūm-therapie wordt meestal uitgevoerd bij de praktijk van uw uroloog of op een polikliniek en wordt in één korte afspraak afgerond.
Tijdens elke behandeling van 9 seconden komt steriele waterdamp vrij in het beoogde prostaatweefsel. Wanneer de stoom in contact komt met het prostaatweefsel, komt alle opgeslagen energie vrij in het weefsel. Uw arts zal bepalen hoeveel behandelingen u nodig heeft, op basis van de grootte van uw prostaat.
Na verloop van tijd absorbeert de natuurlijke genezingsreactie van uw lichaam het behandelde weefsel, waardoor de prostaat kleiner wordt. Met het extra weefsel verwijderd, gaat de urethra open, waardoor de BPH-symptomen worden verminderd.

    Rezum is a minimally invasive transurethral water vapour therapy for benign prostatic enlargement which uses thermal energy for treatment. The short-term results show it to have good outcomes with a potential for outpatient-based treatment preserving sexual function. This review serves to provide an overview of the technique and evaluate its safety and efficacy.

    Large, multi-center, prospective registry of Rezūm water vapor therapy for benign prostatic hyperplasia

    ABSTRACT

    Objective

    : To illustrate the role of Rezūm convective water vapor ablation for treatment of lower urinary tract symptoms secondary to BPH.

    Methods

    : A prospective registry was established for Rezūm therapy in Canada at two high-volume centers. All patients had baseline medical and BPH history documented, along with uroflowmetry and validated questionnaires up to 12 months post-surgery.

    Results

    : 229 patients (mean age 67.3 years), including 83 with prostate volumes ≥ 80 mL, were treated from April 2019 to December 2020. The mean prostate volume was 71.5mL (range 20-160mL) and 55% had a median lobe. The mean number of injections was 11 (range: 4-28) and mean procedural length was 4.8 minutes (range: 1.5-14). The mean duration of post-procedure catheterization was 9.8 days. IPSS scores improved from baseline by 29%, 53%, and 59% at 1, 3, and 12 months, respectively. IPSS QoL improved from baseline by 30%, 50%, and 67% at 1, 3, and 12 months, respectively. Qmax improved by 60% at 3 months and 74% at 12 months. PVR improved by 51% and 61% at 3 and 12 months, respectively. No statistically significant changes were seen in IIEF-15 or MSHQ-EjD scores. No Clavien-Dindo events ≥ Grade III occurred.

    Conclusion

    : Rezūm therapy is a safe, effective, and quick outpatient procedure for prostate glands over a wide range of volumes. Clinically significant improvements are seen in all validated questionnaires. Objective maximum flow measures improved, while erectile and ejaculatory function remains preserved.
