12 augustus 2011: inmiddels is er wel een en ander veranderd ten aanzien van met name Avastin - bevacizumab bij darmkanker. Toch lijkt het me goed om onderstaand artikel te laten staan al is het alleen maar om te zien hoe de ontwikkeling is geweest van bepaalde medicijnen.

Volgens wetenschappers hebben darmkankerpatiënten in 50% tot soms 70% van de gevallen een overexpressie van de zogeheten EGRF = epidermale groeifactorreceptor. Monoklonale middelen als Tarceva en Iressa bij longkanker worden daarvoor ingezet, maar ook in het geval van darmkanker wordt Avastin - Bevacizumab en ook Panitumumab, ook monoklonale middelen, daarvoor ingezet om deze groeifactorreceptor uit te schakelen en te blokkeren waardoor de kankercellen niet meer groeien en op den duur zichzelf doden. Gisteren werd bekend gemaakt dat het grootste biotechbedrijf ter wereld Amgen de producent van Panitimumab Abgenix gaat overnemen. Juist omdat zij verwachten dat Panitumumab wel eens een groot succes zou kunnen worden. Met een verwachte miljarden winst de komende jaren. Met dank aan Willem die ons het persbericht hierover toestuurde. Onderaan dit persbericht hebben we enkele abstracten van studies met panitumumab en met Avastin - bevacizumab een in een fase III studie bewezen monoklonaal middel geplaatst. waaronder een artikel uit het Nederlands Tijdschrift voor Geneeskunde. Antilichaam van twee miljard

Amgen, het grootste biotechbedrijf ter wereld, wil branchegenoot Abgenix overnemen voor 2,2 miljard dollar in contanten. Ook de schulden worden overgenomen. De aandeelhouders van Abgenix moeten het bod, dat 54 procent boven de koers van afgelopen woensdag ligt, nog wel goedkeuren. Het is Amgen vooral te doen om panitumumab, een innovatief kankermedicijn dat bij Abgenix in de pijplijn zit. Panitumumab is een monoklonaal antilichaam dat de epidermale groeifactorreceptor blokkeert. Bij patiënten met uitgezaaide darmkanker, die niet reageerden op conventionele chemotherapie, was het effect dat de tumoren gemiddeld 46 procent trager groeiden. Een dezer dagen wordt het middel voorgelegd aan de FDA ter goedkeuring. Verwacht wordt dat dat niet al te veel problemen oplevert. Daarna ligt een miljardenomzet in het verschiet, vooral als het middel ook blijkt te werken tegen andere vormen van kanker, en in een eerder stadium van de ziekte wordt voorgeschreven. Dankzij de overname van Immunex in 2002 was Amgen al nauw betrokken bij de ontwikkeling van panitumamab. Door ook Abgenix over te nemen, voorkomt het bedrijf dat het straks royalties moet betalen. Het wordt overigens het eerste kankermedicijn in het portfolio. [Chemotherapy of patients with colorectal carcinoma]

Colorectal cancer is a frequently occurring malignancy in the western world. In the Netherlands, there are more than 9000 new patients annually. Approximately half of the patients die of their disease within 5 years. In 2004, several therapeutic studies were presented, the results of which may have a positive impact on the prognosis of a large proportion of patients. This concerns the adjuvant treatment of stage II and III colon carcinoma and the palliative systemic treatment of distant metastases of colorectal carcinoma. In stage II colon carcinoma, the absolute benefit of adjuvant treatment is 3-4%. This must be balanced against the burden of such treatment. Adjuvant treatment with fluorouracil-folinic acid-oxaliplatin is indicated in stage III colon carcinoma and should be considered for patients with stage II colon cancer in whom the prognosis is unfavourable. For patients for whom treatment with fluorouracil-folinic acid-oxaliplatin does not seem suitable, adjuvant treatment with capecitabine is indicated. Treatment with bevacizumab, a monoclonal antibody against vascular endothelial growth factor, in combination with chemotherapy is considered to be standard practice in first-line treatment. Together with the expected rise in the incidence of colorectal cancer, these developments will have a significant impact on healthcare, both in terms of organization and budget.

Overview of monoclonal antibodies and small molecules targeting the epidermal growth factor receptor pathway in colorectal cancer.

The epidermal growth factor receptor (EGFR) provides survival signals and is overexpressed in the majority of colorectal cancers. As more is learned about the molecular details of EGFR signaling, antibodies can be designed to interfere with specific domains of the EGFR molecule. In this review, we analyze preclinical and current clinical data on EGFR-targeting molecules and their potential role in the treatment of colorectal cancer. Cetuximab binds to domain III of EGFR and hinders ligand binding. It is now approved by the US Food and Drug Administration for metastatic colorectal cancer treatment. Panitumumab is another widely studied anti-EGFR antibody with similar properties. Bispecific antibodies are modified immunoglobulin molecules containing 2 different binding specificities. These antibodies can redirect the immune response against tumor cells by tethering effector cells such as CD3e-expressing T cells or CD16-expressing natural killer cells and granulocytes to the surface of cancer cells. Tyrosine kinase inhibitors are quinazoline-derived, low molecular weight synthetic molecules that can block the intracellular tyrosine kinase domain of several receptors, including EGFR, Erb2, and vascular endothelial growth factor receptor, and thereby inhibit ligand-induced receptor phosphorylation and abrogate the biologic effect of EGFR signaling. The presence of skin rash and EGFR gene amplification have been advanced as possible predictors of clinical effectiveness of targeted anti-EGFR therapies.

Targeted therapies in the treatment of colorectal cancers.

Medical Oncology/Hematology Program, H. Lee Moffitt Cancer Center & Research Institute, Tampa, FL 33612-9797, USA. BACKGROUND: In solid organ malignancies, no tumor type has seen a greater impact from the development of novel targeted therapies in 2004 than metastatic colorectal cancer.

METHODS: We review the current progress to date with the use of monoclonal antibodies in colorectal cancer and look at newer therapies under investigation.

RESULTS: Two monoclonal antibodies received Food and Drug Administration approval in early 2004, both for the indication of advanced, metastatic colorectal cancer. A large, randomized, placebo-controlled study demonstrated that the addition of a monoclonal antibody to vascular endothelial growth factor, bevacizumab, led to a statistically significant improvement in overall survival, with tolerable additional toxicity. Chimeric monoclonal antibody therapy directed at the epidermal growth factor receptor was associated with radiographic responses in a significant minority of patients with irinotecan-refractory colon cancer in a randomized phase II study of patients with irinotecan-refractory disease.

CONCLUSIONS: These dramatic successes have led to further clinical studies of targeted therapy in colorectal cancer, making it one of the most promising areas of cancer research.
