Wie besmet is met het HPV virus heeft een groter risico op het krijgen van huidkanker met het plaveisel carcinoom type. Dit blijkt uit een onderzoek onder ca. 4000 mensen met huidkanker, in de leeftijd tussen 24 en 72 jaar. Al eerder is aangetoond dat het HPV virus vaak verantwoordelijk lijkt voor bepaalde vormen van mond- en keelkanker, zie hier reviewstudie daarover. De resultaten werden afgelopen week gepubliceerd in het British Journal of Medicin en door Medscape voorzien van commentaar.

In deze studie onderzochten Dr Karagas en collega's de HPV-status van 2366 patiënten en controles (663 mensen met een plaveiselcelcarcinoom, 898 mensen met een basaalcelcarcinoom, 805 personen in een controlegroep zonder kanker). Alle deelnemers aan de studie waren tussen de 25 en 74 jaar.

De auteurs constateerden dat mannen een iets groter deel van antilichamen voor geslachten β positieve HPV-typen hadden dan vrouwen, maar het verschil was niet statistisch significant (48% bij mannen versus 42% bij vrouwen; P = 0,11). De studie werd gecorrigeerd naar leeftijd, opleidingsniveau, rookgedrag, gevoeligheid van de huid voor de zon, of het aantal pijnlijke zonnebrandplekken.

Zij merkten op dat de OR voor het plaveiselcelcarcinoom verhoogd was voor elk van de HPV-typen die zijn onderzocht, maar er was geen correlatie met basaalcelcarcinoom. Zij vonden ook dat ten minste 1 genus β HPV type positief werd geassocieerd met  0,97 (95% CI, 0,80 tot 1,19) voor basaalcelcarcinoom en van 1,30 (95% CI, 1,04 tot 1,61) voor plaveiselcelcarcinoom .

De auteurs merken op dat zij "een duidelijk stijgende trend zien in het risico voor het plaveiselcelcarcinoom met het toenemen van het aantal types van het  humaan papillomavirus bij de patienten." De UPR's waren als volgt:

Voor 1 positief type, de OR was 0,99 (95% CI, 0,74 tot 1,33)
Voor 2 of 3 positieve typen, de OR was 1,44 (95% CI, 1,03 tot 2,01)
Voor 4 tot 8 positieve typen, de OR was 1,51 (95% CI, 1,03 tot 2,20)
Voor meer dan 8 positieve typen, de OR was 1,71 (95% CI, 1,12 tot 2,62).
De UPR's voor het plaveiselcelcarcinoom waren iets hoger bij patiënten met histologisch bewijs van actinische keratose (OR, 1.59; 95% BI, 1,03 tot 2,44) dan bij degenen zonder. Zij vonden ook dat er een sterkere associatie was tussen HPV en plaveiselcelcarcinoom bij patiënten met een zon-gevoelig fenotype, echter er was geen associatie met een geschiedenis van zonnebrandplekken.


De studie werd gefinancierd door subsidies in het kader van de Amerikaanse National Institutes of Health, het National Cancer Institute en de Europese Gemeenschap. De auteurs hebben meegedeeld geen relevante financiële relaties te hebben.

Positivity to HPV Raises the Risk

In this study, Dr. Karagas and colleagues were able to obtain information on the HPV status of 2366 cases and controls (663 people with squamous cell carcinoma, 898 people with basal cell carcinoma, 805 control subjects). All study participants were residents of New Hampshire and were between 25 and 74 years of age.

The authors found that men had a slightly higher proportion of antibodies positive for genus β HPV types than women, but the difference was not statistically significant (48% in men vs 42% in women; P = .11). However, no trends were observed in antibody prevalence when the cohort was analyzed by age, level of education, smoking status, skin sensitivity to the sun, or number of painful sunburns.

They observed that the OR for squamous cell carcinoma was elevated for each of the HPV types that were examined, but there was no correlation with basal cell carcinoma. They also found that positivity to at least 1 genus β HPV type was associated with an overall OR of 0.97 (95% CI, 0.80 - 1.19) for basal cell carcinoma and of 1.30 (95% CI, 1.04 - 1.61) for squamous cell carcinoma.

The authors note that they "found a clear increasing trend in risk of squamous cell carcinoma with increasing number of human papillomavirus types positive." The ORs were as follows:

  • For 1 positive type, the OR was 0.99 (95% CI, 0.74 - 1.33)
  • For 2 or 3 positive types, OR was 1.44 (95% CI, 1.03 - 2.01)
  • For 4 to 8 positive types, OR was 1.51 (95% CI, 1.03 - 2.20)
  • For more than 8 positive types, OR was 1.71 (95% CI, 1.12 - 2.62).

The ORs for squamous cell carcinoma were slightly higher in patients with histologic evidence of actinic keratoses (OR, 1.59; 95% CI,1.03 - 2.44) than in those without it. They also found that there was a stronger association between HPV and squamous cell carcinoma among patients with a sun-sensitive phenotype, although there was no association with a history of sunburns.

Dr. Karagas plans on continuing research in this area. "There are a wide range of cutaneous human papillomaviruses in addition to those in genus β," she said. "We would like to explore whether other cutaneous human papillomaviruses are associated with risk of nonmelanoma skin cancers."
