Regorafenib blijkt bij vergevorderde maagkanker die al enkele chemokuren achter de rug hebben de progressievrij tijd en overall overleving te verlengen. Al zijn de cijfers nou niet echt indrukwekkend, al moeten we in ogenschouw nemen dat deze patiëntengroep in principe uitbehandeld was. Dan is progressievrij tijd mediaan van 0,9 maanden naar 2,6 maanden en mediane overall overleving van 4,4 naar 5,8 maanden misschien weer wel positief procentueel gesproken. Deze resultaten worden ook als statistisch significant gezien.  

Kernpunten zijn:

  • Regorafenib verlengde significant progressie vrije overleving bij patiënten met vooraf behandelde gevorderde maagkanker. 
  • De resultaten waren in Zuid-Korea beter dan in andere landen. Maar voor alle andere factoren gecorrigeerd gelden dezelfde criteria en resultaten.
  • Bijwerkingeprofiel was voor regorafenib wel iets groter dan voor de placebo / beste zorggroep. Maar goed behandelbaar.
  • Een fase III studie is gepland.

Resultaten:

147 evalueerbare patiënten met 1 of 2 chemokuren eerder gehad voor gevorderde maagkanker in ziekenhuizen in Canada, Australië, Nieuw Zeeland en Zuid Korea werden gerandomiseerd ingedeeld op een 2:1 basis in de periode van november 2012 en february 2014.

  • De ene groep (N=50) ontving op de dagen 1 t/m 21 van een 28 dagen cyclus beste zorg plus een placebo.
  • De andere groep (N=97) beste zorg plus regorafinib oraal 160 mg.
  • De behandeling ging door tot de ziekte progressie vertoonde of tot de bijwerkingen onacceptabel zouden zijn.
  • Doel was progressie vrije ziekte te vergelijken
  • Patienten hadden een leeftijd van gemiddeld 62 tot 63 jaar,
  • 80% in beide groepen waren man.
  • 42% aen 58% in beide groepen hadden al eerder 1 of 2 chemokuren gehad. 
  • 37% van de patienten kwam uit Zuid Korea, 52% uit Australië of Nieuw Zeeland, en 11% uit Canada.

Progressie-vrije overleving:

Mediane progressei vrije tijd was 2.6 maanden (95% confidence interval = 1.8–3.1 maanden) in de regorafenib groep vs 0.9 maanden (95% CI = 0.9–0.9 months) in de placebo group (hazard ratio = 0.40, P < .001).
De behandelingsresultaten waren groter bij de patiënten uit Zuid Korea (HR = 0.12, P < .001) dan bij de patiënten uit Australië, Nieuw Zeeland en Canada (HR = 0.61, P = .03; P < .001 voor interactie); verder waren de resultaten dezelfde en consistent  gerelateerd aan leeftijd, neutrophil-to-lymphocyte ratio, primaire tumor plaats, aantal chemokuren vooraf, aanwezigheid van peritoneale uitzaaiingen (op buikvlies), aantal uitzaaiingen en EGFR waarden.

Mediane overall overleving was 5.8 vs 4.5 maanden (HR = 0.74, P = .147). Na progressie van de ziekte in de placebogroep kregen alsnog 29 patiënten regorafenib.

Graad ≥ 3 bijwerkingen traden op bij respectievelijk 67% vs 52% van de patiënten, met de belangrijkste bijwerking in de regofarenib groep te hoge bloeddruk. (10% vs 2%), verhoogde ASAT waarden (9% vs 0%), en ALAT waarden (8% vs 6%). Ernstige bijwerkingen deden zich voor bij 32% vs 18% van de patiënten, met de belangrijkste maagstoornissen (11% vs 0%) en infecties (6% vs 2%).

Conclusie: regorafenib was in deze studie effectief in verlenging van progressie vrije ziekte bij gevorderde maagkanker.

Abstract

Purpose We evaluated the activity of regorafenib, an oral multikinase inhibitor, in advanced gastric adenocarcinoma.

Patients and Methods We conducted an international (Australia and New Zealand, South Korea, and Canada) randomized phase II trial in which patients were randomly assigned at a two-to-one ratio and stratified by lines of prior chemotherapy for advanced disease (one v two) and region. Eligible patients received best supportive care plus regorafenib 160 mg or matching placebo orally on days 1 to 21 of each 28-day cycle until disease progression or prohibitive adverse events occurred. The primary end point was progression-free survival (PFS). Final analysis included data to December 31, 2014.

Results A total of 152 patients were randomly assigned from November 7, 2012, to February 25, 2014, yielding 147 evaluable patients (regorafenib, n = 97; placebo, n = 50). Baseline characteristics were balanced. Median PFS significantly differed between groups (regorafenib, 2.6 months; 95% CI, 1.8 to 3.1 and placebo, 0.9 months; 95% CI, 0.9 to 0.9; hazard ratio , 0.40; 95% CI, 0.28 to 0.59; P < .001). The effect was greater in South Korea than in Australia, New Zealand, and Canada combined (HR, 0.12 v 0.61; interaction P < .001) but consistent across age, neutrophil-to-lymphocyte ratio, primary site, lines of chemotherapy, peritoneal metastasis presence, number of metastatic sites, and plasma vascular endothelial growth factor A. A survival trend in favor of regorafenib was seen (median, 5.8 months; 95% CI, 4.4 to 6.8 v 4.5 months; 95% CI, 3.4 to 5.2; HR, 0.74; P = .147). Twenty-nine patients assigned to placebo received open-label regorafenib after disease progression. Regorafenib toxicity was similar to that previously reported.

Conclusion In this phase II trial, regorafenib was effective in prolonging PFS in refractory advanced gastric adenocarcinoma. Regional differences were found, but regorafenib was effective in both regional groups. A phase III trial is planned.
