Velcade - Bortezomib toont hoopgevende therapeutische waarde bij patiënten met een recidief van mantelcel lympfomen en daarnaast ook bemoedigende resultaten bij patiënten met recidief van andere B-cel lympfomen. Dit blijkt uit een fase II studie bij 60 patiënten verdeeld in twee groepen, te weten groep A. met mantel cel lympfoom patiënten en groep B. met patiënten met andere B-cel lympfomen, zoals focculaire non-Hodgkin, ziekte van Waldenström enz. We vertaalden voor u het abstract zo goed als letterlijk:

PATIENTEN EN METHODES: Patiënten werden verdeeld gebaseerd op preclinische data, in arm A (mantel-cell lymphoma's) of arm B (andere B-cell lymphoma's) zonder beperking van aantal van vooraf gekregen andere behandelingen. Bortezomib - Velcade werd toegediend als een intraveneuze injectie(1.5 mg/m2) op de dagen 1, 4, 8, en 11 ieder 21 dagen voor een maximum van 6 cycli

RESULTATEN: 60 patiënten met een gemiddeld aantal van eerdere behandelingen van 3.5 (range, een tot 12 behandelingen) werden toegelaten; 33 patiënten werden in arm A geplaatst en 27 werden in arm B geplaatst, inclusief 12 diffuse grote B-cell lymphomas, 5 follicular lymphomas (FL), 3 getransformeerde follicular lymphomas - FLs, vier kleine lymphocytische lymphomas (SLL), twee ziekte van Waldenström (WM) patiënten, en een marginale zone lymphoma. In arm A, reageerden 12 van de evalueerbare patiënten op de behandeling (6 complete remissies en 6 gedeeltelijke remissies ) met een overall response cijfer (ORR) van 41% (95% CI, 24% tot 61%), en een mediane tijd tot progressie (ziektevrije tijd) is nog niet bereikt, met een mediane follow-up van 9.3 maanden (range, 1.7 tot 24 maanden). In arm B, vier van de 21 evalueerbare patiënten reageerde op de behandeling (een SLL patient had een complete remissie - CR, een FL patiënt had a CR unconfirmed, one diffuse large B-cell lymphoma patiënt had a PR, en een ziekte van Waldenström patiënt had een gedeelteljike remissie - PR) met een voerall response cijfer - ORR van 19% (95% CI, 5% tot 42%). Grade 3 toxiciteit inclusief thrombocytopenia (47%), spijsverteringsproblemen (20%), moeheid (13%), neutropenia (10%), en peripheral neuropathy (5%). Grade 4 toxiciteit kwam voor bij 9 patiënten (15%), en drie sterfgevallen door progressie van de ziekte binnen 30 dagen na beeindigng van de behandeling.

CONCLUSIE: Bortezomib toont hoopgevende activiteit bij patiënten met een recidief van mantel-cell lymphoma en toont bemoedigende resultaten bij andere B-cell lymphoma's. Verdere studies zullen bortezomib onderzoeken in combinatie met andere cytotoxische of biologische agenten - medicijnen.

PURPOSE: Evaluate efficacy and toxicity of bortezomib in patients with relapsed or refractory B-cell non-Hodgkin's lymphoma.

PATIENTS AND METHODS: Patients were stratified, based on preclinical data, into arm A (mantle-cell lymphoma) or arm B (other B-cell lymphomas) without limitation in number of prior therapies. Bortezomib was administered as an intravenous push (1.5 mg/m2) on days 1, 4, 8, and 11 every 21 days for a maximum of six cycles.

RESULTS: Sixty patients with a median number of prior therapies of 3.5 (range, one to 12 therapies) were enrolled; 33 patients were in arm A and 27 were in arm B, including 12 diffuse large B-cell lymphomas, five follicular lymphomas (FL), three transformed FLs, four small lymphocytic lymphomas (SLL), two Waldenstrom's macroglobulinemias (WM), and one marginal zone lymphoma. In arm A, 12 of 29 assessable patients responded (six complete responses and six partial responses ) for an overall response rate (ORR) of 41% (95% CI, 24% to 61%), and a median time to progression not reached yet, with a median follow-up of 9.3 months (range, 1.7 to 24 months). In arm B, four of 21 assessable patients responded (one SLL patient had a CR, one FL patient had a CR unconfirmed, one diffuse large B-cell lymphoma patient had a PR, and one WM patient had a PR) for an ORR of 19% (95% CI, 5% to 42%). Grade 3 toxicity included thrombocytopenia (47%), gastrointestinal (20%), fatigue (13%), neutropenia (10%), and peripheral neuropathy (5%). Grade 4 toxicity occurred in nine patients (15%), and three deaths from progression of disease occurred within 30 days of withdrawal from study.

CONCLUSION: Bortezomib showed promising activity in relapsed mantle-cell lymphoma and encouraging results in other B-cell lymphomas. Future studies will explore bortezomib in combination with other cytotoxic or biologic agents.
