17 februari 2014: toegevoegd een zogeheten case report studie van een 65 jarige man man met onbehandelbare primaire levertumoren die naast een perfusie (lokale chemo) via een portocath systeem toch ook met de nanoknife kon worden geholpen en alsnog klinisch kankervrij werd. Nanoknife - IRE - Irreversible electroporation blijkt succesvolle aanpak bij patiënten met inoperabele, onbehandelbare levertumoren. Bij 70 procent van deelnemende patiënten werden alle tumoren volledig vernietigd. Dit presenteerden  onderzoekers van de Universitätsklinikum Regensberg Regensberg, Duitsland op het ARRS congres op 18 april 2013.

De studie werd uitgevoerd bij 22 patiënten die niet behandeld konden worden met een andere behandeling zoals TACE - Trans Arteriële Chemo Embolisatie of RFA - Radio Frequency Ablation  bv. omdat hun tumoren op een plaats in de lever zat waar geen behandeling mogelijk was. Bv. tumoren lagen te dicht bij de hoofdslagader. Met de nanoknife - IRE - Irreversible electroporation bleek 70 procent van de tumoren volledig te zijn vernietigd. Dr Philipp Wiggermann, hoofdauteur van de studie, zegt: "Als men bedenkt dat IRE echt de enige optie was voor deze patiënten, dan zijn deze resultaten veelbelovend."

Er waren bij 2 patiënten twee ernstige complicaties tijdens het onderzoek, maar geen was levensbedreigend, zegt Dr Wiggermann. "Bij een patiënt deed zich een partiële trombose voor van de linker poortader." Het is niet duidelijk of de IRE procedure dit veroorzaakte, maar omdat deze gedeeltelijke trombose zich voordeed na de ablatie behandeling beschouwen we dit als een behandelings gerelateerde bijwerking," aldus dr. Wiggeremann. Ook was er een incident waarbij letsel aan de galblaas ontstond tijdens de ablatie behandeling. "Toevallige schade aan omringende organen is een risico van percutane ablatie technieken. Behandeling van levertumoren is bijzonder moeilijk. Vanwege de ademhaling van het membraan leidt dit tot een voortdurende verschuiving van de orgaan positie" zo zegt Dr Wiggermann.

IRE - Irreversible electroporation verstoort de celmembraan en leidt automatisch tot celdood. De nanoknife - IRE - Irreversible electroporation methode wordt momenteel klinisch onderzocht in verschillende landen voor de behandeling van kwaadaardige levertumoren en longlaesies, aldus Dr Wiggermann (zie ook gerelateerde artikelen voor studies in Nederland met de nanoknife) . New ablation technique holds promise for liver cancer patients. When standard treatment doesn't work or isn't possible

A new minimally invasive tumor ablation technique is providing hope for liver cancer patients who can't undergo surgery or thermal ablation, a study shows.

The study of 22 patients at the Universitatsklinikum Regensberg in Regensberg, Germany, found that irreversible electroporation (IRE) successfully destroyed tumor tissue in 70% of these patients. These patients were not responsive to conventional therapy or their tumor was in a location that was not suitable for standard treatment, said Dr. Philipp Wiggermann, lead author of the study. "If one considers that IRE was really the only option for these patients, the results are very promising," he said.

There were two major complications in the study, but neither of them was life-threatening, said Dr. Wiggermann. One patient suffered partial thrombosis of the left portal vein. "It is not clear that the IRE procedure caused the partial thrombosis, but since it appeared after the ablation treatment, it was considered as a therapy-associated side effect," he said. There was accidental injury to another patient's gallbladder during the ablation treatment. "Accidental injury to surrounding organs is a risk of all percutaneous ablation techniques. Treatment of liver tumors is especially difficult due to the respiratory movement of the diaphragm leading to continuous shifting of the organ position," said Dr. Wiggermann.

IRE disrupts the cell membrane and results in cell death. It is currently undergoing clinical investigation for treatment of malignant liver and lung lesions, Dr. Wiggermann said.

Dr. Wiggermann's study will be presented April 18 at the ARRS Annual Meeting in Washington, DC.

Irreversible electroporation of a hepatocellular carcinoma lesion adjacent to a transjugular intrahepatic portosystemic shunt stent graft.

Abstract

We report in a 65-year-old man hepatocellular carcinoma adjacent to a transjugular intrahepatic portosystemic shunt stent-graft which was successfully treated with irreversible electroporation (IRE). IRE is a new non-thermal tissue ablation technique which uses electrical pulses to induce cell necrosis by irreversible membrane poration. IRE proved to be more advantageous in the ablation of perivascular tumor with little injury to the surrounding structures.

Abstract

Purpose:
The aim of this study is to describe the image findings of contrast-enhanced ultrasound (CEUS) after irreversible electroporation (IRE), and to assess its usefulness in relation to the evaluation of the ablation status using a dynamic recording of the microvascularisation.

Material and method:
In this prospectively designed study, a percutaneous IRE was performed for the treatment of malignant hepatic lesions in a total of 15 patients following the interdisciplinary diagnosis. The lesions were documented using CEUS before, immediately after and 20 minutes after the ablation. The acquired CEUS image data was subsequently independently retrospectively evaluated by 2 experienced radiologists and assessed in relation to microvascularisation using a 5 point scale.

Results:
Using CEUS, a significant reduction in the microcirculation of the lesions, both centrally and marginally, could be detected following IRE (p < 0.001 and p < 0.001). The mean evaluation of the central microcirculation was reduced from 3.13 ± 1.38 to 0.47 ± 0.64 (p < 0.01). For the peripheral area of the lesion and the ablation margin, there was a reduction of 3.37 ± 1.13 to 1.57 ± 0.46 (p = 0.001). There was no significant reduction in the macrocirculation.

Conclusion:
Within the framework of this study, it could be demonstrated that, in the course of IRE, a rapid significant reduction of the microcirculation in the ablation area occurred.
