Wanneer patiënten met diabetes type 1 na een stamceltransplantaite het anti-CD40L antilichaam Tegoprubart krijgen dan hebben zij grote kans geen insuline meer te hoeven gebruiken. Dat blijkt uit de resultaten van een fase 1 studie met 12 patiënten met diabetes type 1. 

Alle deelnemende 12 patiënten in de studie bereikten insuline-onafhankelijkheid; zij produceerden na de behandelingen hun eigen insuline en hadden geen extra insulinetherapie meer nodig voor de behandeling van hun diabetes type 1.
– Alle 12 patiënten bereikten bovendien een HbA1c-waarde van minder dan 6,5%, met een gemiddelde meest recente HbA1c-waarde van ongeveer 5,4%; dit komt overeen met een gemiddelde verbetering van ongeveer 2,6% ten opzichte van de uitgangswaarde
– Na de stamceltransplantatie werden geen periodes met ernstige te lage bloedsuikerwaarden gemeld, in tegenstelling tot de voorgeschiedenis van alle deelnemende patiënten, die vóór de stamceltransplantatie terugkerende ernstige te lage bloedsuikerwaarden voorvallen hadden doorgemaakt.

IRVINE, Calif., June 08, 2026 (GLOBE NEWSWIRE) -- Eledon Pharmaceuticals, Inc. (“Eledon”) (NASDAQ: ELDN) today announced updated results from an investigator-initiated trial evaluating tegoprubart, its investigational anti-CD40L antibody, as part of a calcineurin inhibitor-free immunosuppression regimen in patients with type 1 diabetes undergoing allogeneic islet cell transplantation at the University of Chicago Medicine Transplant Institute. The results were presented by trial investigator Piotr Witkowski, M.D., Ph.D., Director of the Pancreas and Islet Transplant Program at UChicago Medicine, at the American Diabetes Association 86th Scientific Sessions, taking place June 5-9, 2026, in New Orleans, Louisiana.

All patients treated in the study (n=12) showed rapid improvement in glycemic control following islet transplantation and treatment with tegoprubart, with stable islet graft function observed across the cohort over a median and maximum post-transplant follow-up period of 8 and 22 months, respectively. All 12 patients achieved insulin independence, meaning that they no longer needed chronic, exogenous insulin therapy to manage their T1D. Purpose: Beta cell replacement therapy has potential to reverse diabetes, but toxicity of immunosuppression medications limits its application. Tegoprubart (Eledon Pharmaceuticals), a novel monoclonal antibody targeting CD40 ligand (CD40L), offers a potential alternative with minimal toxicity. This study evaluates the safety and efficacy of Tegoprubart in patients receiving cadaveric islet transplantation. Methods: Six patients with type 1 diabetes and problematic hypoglycemia have been enrolled in this single-center, phase 1/2 clinical trial. Induction immunosuppression included anti-thymocyte globulin, followed by tegoprubart (20 mg/kg IV every three weeks after an initial loading dose) and mycophenolate for maintenance therapy. Results: Patient #1 with BMI of 29 reduced insulin requirements from 80 to 30 units/day after a single-donor intraportal islet transplantation (363,000 IEQ, 4,092 IEQ/kg). A second transplant led to insulin independence still sustained for 11 months. Patient #2 with BMI 20 achieved insulin independence one month after a single-donor islet transplantation (326,000 IEQ, 6,700 IEQ/kg). She has remained insulin-free for over a year with stable graft function with A1c of 5.1. Patient #3 with BMI of 30 maintained stable islet graft function and reduced HbA1c from 9.9% to 6.4% three months after his second islet transplant. Patient #4 and #5 stopped her insulin support 3 weeks after her first islet transplantation and maintain optimal blood glucose control with HbA1c decreased to 5.9% at 6 week follow up. Patient 6# is recovering from his first islet transplant requiring reduced insulin support with HbA1c already 6.0%. All patients had an uncomplicated post-transplant course, with no adverse events, donor-specific antibodies, or immunologic rejection. No opportunistic infections occurred. Conclusions: Preliminary data from the first six patients indicate that Tegoprubart with mycophenolate is well-tolerated and effective in preventing islet graft loss, with no unexpected adverse events. Further observations are warranted to validate these findings.
