Korte sessies van PEMF-therapie - niet-invasieve gepulste elektromagnetische velden - verandert de zogeheten TAM's = Tumor geAssocieerde Macrofagen van cellen die de kwaadaardige tumor doen groeien (M2) in immuuncellen die kankercellen vernietigen (M1) bij borstkankerpatiënten. Bij PEMF-therapie worden gedurende korte tijd intermitterende magnetische pulsen met lage intensiteit op specifieke lichaamsgebieden toegepast.
In hun meest recente studie toonden de onderzoekers van het NUS aan dat PEMF-therapie – zelfs zonder chemotherapie – tumoren volledig konden elimineren in 75 procent van de geteste preklinische modellen, na slechts vier sessies van 30 minuten.
Belangrijkste punten uit de studie:
- PEMF-therapie herprogrammeert macrofagen naar de M1-toestand.
- PEMF-therapie activeert de STING-signaalroute op een TRPC1-afhankelijke wijze.
- PEMF-therapie verhoogt de immunogeniciteit van kanker.
- PEMF-therapie moduleert de paracriene interactie tussen macrofagen en kankercellen.
- PEMF-therapie versterkt de anti-kankerimmuniteit door in vitro, ex vivo en in vivo de door macrofagen gemedieerde, kankerspecifieke fagocytose te bevorderen.
De wetenschappelijke studie, zie hier het studieprotocol, betrof een 'single-center', open-label fase Ib-studie met een '3+3'-opzet voor dosis-escalatie. Patiënten met histologisch bevestigde borstkanker type ductaal carcinoom werden ingedeeld in drie opeenvolgende studiegroepen.
- Voor dosisniveau 1 werden patiënten met uitgezaaide ziekte (graad 3, stadium III/IV, ER-/HER2+) geselecteerd voor één PEMF-sessie van 30 minuten, om de lokale toxiciteit te beoordelen.
- Voor dosisniveau 2 werden patiënten geselecteerd bij wie een chirurgische ingreep gepland stond (graad 1-2, ER+/PR+), om de veiligheid met betrekking tot wondgenezing te evalueren; PEMF werd 30 minuten voorafgaand aan de operatie toegediend.
- Binnen dosisniveau 3 werden patiënten geïncludeerd die neoadjuvante chemotherapie ondergingen (graad 3, stadium I-III); zij kregen PEMF toegediend 30 minuten voor hun laatste cyclus (4e cyclus) van een behandeling met ofwel chemo met antracyclines ofwel chemo met taxanen.
- De primaire doelen waren veiligheid en de incidentie van bijwerkingen die verband hielden met het hulpmiddel.
- Het onderzoek werd afgerond bij dosisniveau 3 en toont aan dat een PEMF-therapie van 30 minuten veilig is en goed wordt verdragen door borstkankerpatiënten, zowel in de setting van uitgezaaide ziekte als bij pre-operatieve en neoadjuvante chemotherapie.
- De interventie had geen nadelige invloed op de wondgenezing na de operatie of op de toediening van chemotherapie.
De studie, onder leiding van universitair hoofddocent Franco-Obregón van de afdeling Chirurgie van de NUS Yong Loo Lin School of Medicine en het NUS Institute for Health Innovation & Technology, werd op 4 juni 2026 gepubliceerd in het tijdschrift *Smart Medicine*. Deze ontdekking bouwt voort op eerder onderzoek, waarin werd aangetoond dat korte blootstelling aan PEMF-therapie de opname van doxorubicine – een chemo – door borstkankercellen verhoogt.
"Onze studie betekent een belangrijke vooruitgang in de behandeling van borstkanker. Magnetic Reprogramming of Macrophages Stimulates Phagocytosis of Breast Cancer Cells via a TRPC1-STING Inflammatory Axis

ABSTRACT
The reprogramming of tumor-associated macrophages (TAMs) from a pro-tumoral M2 to an anti-tumoral M1 phenotype is an attractive therapeutic strategy whose clinical translation is undermined by the systemic toxicity of currently available pharmacological approaches. Here, we demonstrate that non-invasive and localizable pulsed electromagnetic fields (PEMFs) induce macrophage reprogramming downstream of transient receptor potential canonical 1 (TRPC1) channel activation. Brief (10 min) PEMF exposure polarized macrophages toward an M1 phenotype by activating Stimulator of Interferon Genes (STING)-dependent NF-κB inflammatory pathways that were abolished by TRPC1 knockdown or inhibition. PEMF exposure directly enhanced the immunogenicity of breast cancer cells and modified macrophage-cancer crosstalk to promote M1 macrophage polarization and the attraction of STING-activated macrophages to the cancer cells. In co-cultures, PEMF exposure stimulated macrophage-mediated phagocytosis of cancer cells in a STING- and TRPC1-dependent manner. In spheroids, PEMFs induced the reprogramming of TAMs to an M1 status and selectively enhanced infiltration of M1 macrophages, resulting in STING-mediated phagocytosis of cancer cells. In mice, 2 weeks of twice-weekly PEMF exposure resorbed engrafted tumors and selectively eliminated cancer cells within tumors while promoting immune cell recruitment. PEMFs offer a non-invasive manner to locally reprogram TAMs within the tumor microenvironment to preferentially eliminate cancer cells.
