Nederlandse onderzoekers (Jan Schellens en Rene Bernards ) aan het NKI / AvL hebben ontdekt hoe bij melanomen de resistentie te omzeilen van tumorcellen van het type melanomen die eerder behandeld zijn met zogeheten BRAF-remmers.

Zij gebruikten daarvoor vorinostat een reeds jarenlang bestaand medicijn waarvan het patent is verlopen en maakten dit medicijn vorinostat in eigen beheer (ziekenhuis apotheker Jos Beijnen nam deze nagemaakte vorinostat voor zijn rekening) en Jan Schellens zijn groep deed daarmee een kleine fase I studie die in Cell werd gepubliceerd afgelopen week. Met uitstekende resultaten, eerst bij muizen en daarna bij enkele patiënten met melanomen. Het volledige studierapport in Cell:

Cancer cells that develop resistance to drugs, pay a price for this, by simultaneously developing a new vulnerability.  If this acquired vulnerability can be identified, it may be exploited clinically. A team of cancer researchers, led by Rene Bernards of the Netherlands Cancer Institute and Oncode Institute, now exposed this acquired vulnerability in melanoma that has developed resistance to a targeted therapy with BRAF-inhibitors. The team then developed a new therapeutic strategy to selectively kill the drug-resistant cancer cells.

Vorinostat ook bij ziekte van Kahler - Multiple Myeloma zeer succesvol:

Het is natuurlijk heel mooi nieuws. Maar in de media horen of lezen we alleen dat het Oncode Instituut een nieuwe afdeling binnen het NKI, vorinostat heeft nagemaakt en daarmee dit medicijn dus veel goedkoper kan produceren. Kosten van het medicijn in het AvL zijn 1.500 euro per patiënt per jaar, waar farmaceut Merck bijna 70 keer zoveel voor rekent en uitkomt op ca. €  100.000,-- per patient per jaar.

Wat er niet bij wordt verteld door de onderzoekers van het NKI is dat vorinostat al eerder bij zwaarvoorbehandelde patiënten met vergevorderde multiple myeloma al in een fase I en fase II studie book bijzonder effectief bleek naast de standaard behandelingen van bortezomib, doxorubicin, and dexamethasone (VBDD). Uit een recente publicatie: Safety and efficacy of vorinostat, bortezomib, doxorubicin and dexamethasone in a phase I/II study for relapsed or refractory multiple myeloma (VERUMM study: vorinostat in elderly, relapsed and unfit multiple myeloma). blijkt vorinostat maar liefst 94 procent klinische effectiviteit te geven bij zwaar voorbehandelde patienten met gevorderde mutiple myeloma - ziekte van Kahler.

Uit het studierapport:

The number of prior therapy lines was substantial with a median of 3 (1-9; prior bortezomib treatment: 88%). VBDD was well tolerated with no dose-limiting toxicity: 15 adverse events occurred in 9/33 (27%) patients. The best overall response rate was 67% and clinical benefit rate 94%. With a median follow-up of 30.8 months, median progression-free- and overall-survival were 9.6 and 33.8 months, respectively.

Zou het NKI dit al eerder hebben geweten en hier dus slim op zijn ingesprongen?  Dat Rene Bernards overal vertelt dat zij dit doen in het belang van kankerpatienten ligt misschien toch iets anders en is het financiële belang van het NKI / AvL en Oncode wellicht het belangrijkste belang? Nou ja in ieder geval is het wel een belangrijke ontwikkeling.

Safety and efficacy of vorinostat, bortezomib, doxorubicin and dexamethasone in a phase I/II study for relapsed or refractory multiple myeloma (VERUMM study: vorinostat in elderly, relapsed and unfit multiple myeloma).

Abstract

This phase I/II trial was conducted to investigate the safety, efficacy, and pharmacodynamics of the pan-HDAC-inhibitor (HDACi) vorinostat combined with bortezomib, doxorubicin, and dexamethasone (VBDD) in patients suffering from relapsed/refractory multiple myeloma (RRMM). In the phase I part of this study, 9/33 patients received dose-escalated vorinostat (100, 200, 300mg), using a 4-day-on and 4-day-off schedule, and a standard 3+3 design. In the phase II part of the study, 24/33 patients were included to further assess VBDD's safety and efficacy. Complementary analyses were performed throughout the trial to correlate clinical outcome with patient fitness, pharmacodynamics and potential biomarkers. The number of prior therapy lines was substantial with a median of 3 (1-9; prior bortezomib treatment: 88%). VBDD was well tolerated with no dose-limiting toxicity: 15 adverse events occurred in 9/33 (27%) patients. The best overall response rate was 67% and clinical benefit rate 94%. With a median follow-up of 30.8 months, median progression-free- and overall-survival were 9.6 and 33.8 months, respectively. VBDD-responders showed early pan-HDAC activity decreases in peripheral blood mononuclear cell samples (median 45% of pre-treatment levels), of bone marrow (BM) infiltration rates and of BM-HDAC6 expression. VBDD proved to be an effective, well-tolerated outpatient quadruplet regimen with promising responses in RRMM. Our intermittent HDACi schedule provides a novel treatment option as compared to previous maximum-tolerated-dose-driven HDACi approaches and may serve as a useful example for other HDACi combinations, demonstrating that a continuous epigenetic treatment, with proven synergy to antimyeloma agents, is relevant before HDACi are dismissed as antimyeloma agents.

Summary

BRAF(V600E) mutant melanomas treated with inhibitors of the BRAF and MEK kinases almost invariably develop resistance that is frequently caused by reactivation of the mitogen activated protein kinase (MAPK) pathway. To identify novel treatment options for such patients, we searched for acquired vulnerabilities of MAPK inhibitor-resistant melanomas. We find that resistance to BRAF+MEK inhibitors is associated with increased levels of reactive oxygen species (ROS). Subsequent treatment with the histone deacetylase inhibitor vorinostat suppresses SLC7A11, leading to a lethal increase in the already-elevated levels of ROS in drug-resistant cells. This causes selective apoptotic death of only the drug-resistant tumor cells. Consistently, treatment of BRAF inhibitor-resistant melanoma with vorinostat in mice results in dramatic tumor regression. In a study in patients with advanced BRAF+MEK inhibitor-resistant melanoma, we find that vorinostat can selectively ablate drug-resistant tumor cells, providing clinical proof of concept for the novel therapy identified here.
