Al is het artikel over immuuntherapie: Tumor immunologie: je afweersysteem vecht tegen kanker recenter en ook duidelijker, en aangepast aan de nieuwste ontwikkelingen. Vermindering van ziekteprogressie en opwekking van tumorspecifieke immuunrespons door aanvullende lichaamseigene stoffen ( immuuntherapie) bij melanoom stadium IV.

Melanoom stadium IV is nog steeds een ziekte met een slechte prognose. hoewel moderne chemotherapie en chemo-immuuntherapie een hoge respons percentage (aanslaan van de chemo) laat zien bij stadium IV patiënten, worden remissies (teruggang van de kanker) vaak gevolgd door snelle regressies (de kanker komt weer terug)

Met de bedoeling vroege regressies na chemotherapie bij melanoom stadium IV te voorkomen en in plaats daarvan een stabiel ziekteproces of zelfs een gedeeltelijke of gehele remissie te bewerkstelligen werden de patiënten na de chemo behandeld met 9 millioen IU IFN alpha 5x per week (onderhuids) en 6 millioen IU IL-2 (onderhuids) twee keer per week.

Vergeleken met een controlegroep zonder deze aanvullende behandeling, werd het percentage van ziekteprogressie gereduceerd van maar liefst 95% (controlegroep zonder aanvulling) naar 'slechts' 35% (behandelgroep met aanvullende immuuntherapie)

Ook de tijd wanneer progressie optrad werd significant verlengd. Mediane overlevingstijd in de controlegroep (zonder aanvulling) was 25 weken, terwijl de mediane overlevingstijd in de behandelgroep nog niet is bereikt, (wat wil zeggen dat er op het moment van afsluiten van de trial veel meer mensen nog in leven zijn dan dat er tijdens en na de behandeling zijn overleden)

[Decreased rate of progression and induction of tumor-specific immune response by adjuvant immunotherapy in stage IV melanoma]

Stage IV melanoma is still a disease with poor prognosis. Although modern chemo- or chemoimmunotherapies give high response rates in stage IV patients, remissions are usually followed by fast relapses. In order to avoid early relapses after chemotherapy, patients with stage IV disease and either stable disease or partial or complete remission following therapy were treated with 9 million IU IFN alpha subcutaneously 5 times weekly and 6 million IU IL-2 subcutaneously twice weekly. Compared with untreated controls, the rate of progression in the treatment group was reduced from 95% to 35%. Also, time to progression was significantly prolonged. Median survival times in the control group were 25 weeks, whereas median survival time in the treatment group has not yet been reached. Furthermore, TNF-ELISPOT assays showed a significant increase in MAGE-3 reactive cytotoxic T-cells in the treatment, but not in the control group. Thus, immunotherapy in stage IV disease seems to prolong survival in melanoma patients.

[Adjuvant therapy of primary malignant melanoma with natural human interferon-beta. Significant survival advantage in 96 treated patients in comparison with 288 untreated symptomatic controls]

In the dermatological department of Dortmund's Municipal Medical Centre, between May 1986 and April 1991 a total of 105 patients with primary malignant melanoma (stage I) underwent adjuvant treatment with 5 million IU natural interferon beta as a 30-min i.v. infusion three times weekly for 6 months. During follow-up the patients were examined at short intervals and all recurrences and disease-related cases of death were documented up to September 1992. We evaluated the outcome of patients treated with interferon beta (n = 96 with valid notes of tumour thickness) compared with untreated historical controls (n = 288) matched for tumour thickness, localization, and sex, taken from the Central Malignant Melanoma Registry (CMMR) of the German Dermatological Society. Therefore, the main prognostic factors were identical between cases and controls. A computerized randomization was used to fit three control patients to each treated patient. Survival rate and recurrence-free survival were estimated in both groups for a period of 5 years. During the follow-up 3 patients died in the interferon beta group and the 5-year survival rate was 95%, as against 89% in the control group (P < 0.05 for difference between survival curves). Recurrence-free survival curves were also more favourable for interferon-treated patients than for the control group (P = 0.06). A detailed analysis of high-risk patients with tumour thickness of over 1.5 mm also demonstrated obviously better survival (5 years: 95% vs 77%; P = 0.012) and recurrence-free survival rates (5 years: 75% vs 53%; P < 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)
