28 juni 2012: in mijn zoeken naar studies met tetracycline kwam ik deze studie tegen: Chemically Modified Non-Antimicrobial Tetracyclines are Multifunctional Drugs against Advanced Cancers.  In dit volledige studierapport worden enkele studies met tetracycline bij met name prostaatkanker en bottumoren omschreven en van commentaar voorzien. Voor leken ingewikkeld maar voor artsen wel interessant. Onderaan artikel staat een referentielijst met studies gerelateerd aan tetracyclines.

Abstract

Metastatic cancers account for more than 90% of cancer mortality. The metastasis of all cancers is critically mediated by enzymes that degrade extracellular matrix. Aggressive tumors are characterized by an imbalance between enzymes that degrade ECM and endogenous inhibitors of the enzymes. Matrix metalloproteinases (MMPs) make up the majority of ECM degrading enzymes implicated in cancer metastasis. The potent MMP inhibitory activities of tetracyclines, especially their chemically modified analogs, combined with their relatively well tolerated pharmacological profile, led several researchers to investigate their anticancer potential in a variety of cancers, including melanoma, lung, breast and prostate cancers. Chemically modified non-antibiotic tetracyclines (CMT, or COL) were tested using tumors of prostate, breast and melanomas. Some of these CMTs, notably, CMT-3 and CMT-308 significantly inhibited not only invasive potential and MMP activity, but also inhibited cell proliferation by inducing cell cycle arrest and apoptosis. CMT-3 and CMT-308 were significantly more potent than doxycycline or minocycline in inhibiting tumor cell-derived MMPs and inducing apoptosis in vitro and in vivo. CMT-3 (Col-3) showed potent inhibition of tumor growth in xenografts and in bone metastatic models of prostate cancer. Similar results were also reported in melanoma and breast cancer models. The mechanism by which CMTs kill tumor cells is via generation of hydroxyl free radicals () which permeate and depolarize mitochondria, which in turn activates caspase mediated apoptosis. Analysis of tumor tissues from CMT-3 treated rats demonstrated reduction in angiogenesis and increase in apoptosis; both emerged as mechanisms of CMT action. These observations led to testing the efficacy of CMT-3 in human clinical trials against several types of cancer with significant outcomes, which are described in the next chapter of this issue.

 

31 december 2010: een paar weken geleden vertelde een man die zijn vrouw veruitgezaaide borstkanker heeft/had, dat zijn vrouw veel baat heeft bij tetracycline. Een paar weken na de start van gebruik van tetracycline merkte ze al hele duidelijke verbeteringen, met name in haar botten. Ook een scan later liet zien dat er een duidelijke verbetering was. Of dat door de tetracycline kwam wisten ze niet want ze deden er ook hyperthermie bij. Maar blijft een interessant middel om te volgen lijkt mij.

Tetracycline veelbelovend in het stoppen van kanker

Er is een behandeling tegen kanker ontwikkeld welke veilig, goedkoop en bijna compleet effectief is. Het bestaat al tientallen jaren maar nog nooit heeft iemand eraan gedacht het te gebruiken om deze ziekte te bestrijden. De behandeling is Tetracycline, behorende tot de familie van goedkope Acne antibiotica. Het medicijn is onderzocht op zijn mogelijkheden metastases te voorkomen, de verspreiding van kankertumor cellen welke zich door het lichaam verspreiden, secundaire tumoren veroorzakend.
Dr. Gurmit Singh van het Hamilton Regional Cancer Centre leidt het onderzoek van dit medicijn. Singh ontdekte gedurende de laatste vier jaar dat Tetracycline niet alleen effectief is als bescherming tegen metastases die zich in de botten vormen, het heeft tevens de mogelijkheid om de tumorcellen in de botten te vernietigen.
Bij testen op muizen, ontdekte zijn team dat het medicijn de verspreiding van bepaalde soorten kanker in de botten met 70% belemmerde. “Dit was zeer opwindend voor ons, dus we gingen door met onze experimenten en wat we ontdekten was dat het in feite het bot zelf verbeterde. In sommige gevallen genas het bot” zei Dr. Singh.

Tetracycline werd gebruikt om Acne en Tandwortelziektes te behandelen. Het medicijn wordt opgenomen door tanden en botten en blokkeert een groep enzymen genaamd matrix metalloproteinases, een oorzaak van de achteruitgang van de conditie van het tandvlees. Maar zijn probaatheid in het beletten van de groei van Kanker Osteociasts in botten is nog niet volledig onderzocht. Het probleem kan de prijs van het medicijn zijn, de lage prijs. Omdat het medicijn nogal algemeen en goedkoop is, is er niet veel motivatie van de pharmaceutische industrie om te investeren in onderzoek.

“Pharmaceutische bedrijven, zijn begrijpelijk niet zo bereidwillig om te investeren in ontwikkelingsonderzoek welke niet in een winst voor hun bedrijven zal resulteren,” zegt Dr. Robert Bell van het Princess Margeret Hospital.
Dat is wat Dr. Singh ontdekte toen hij enige pharmaceutische bedrijven benaderde. “Er is geen financiële winst voor de industrie” zij Dr. Singh. “Dit is een algemeen medicijn. Wanneer het een algemeen gepatenteerd medicijn zou zijn dan zou de industrie het steunen.”
Het medicijn heeft de aandacht gekregen van de Canadian Breast Cancer Research Initiative, welke een kleine trial bij 60 borstkankerpatiënten steunt. Dr. Singh is de hoofdonderzoeker van dit project.
Een vrouw, die gekend wil worden als Joyce, vertelde aan Avis Favaro van CTV dat ze van het onderzoek had gehoord, en besloot het te proberen. Haar borstkanker had zich uitgezaaid naar haar botten. Ze voelt zich nu beter.
“Ik begon het te gebruiken en merkte verschillen. Ik had geen pijn meer in mijn rug. Ik kwam uit de rolstoel. Ik begon te lopen……. elke dag, uiteindelijk liep ik zes blokken.
Het heeft me een kans gegeven en ik denk dat anderen die mogelijkheid ook moet worden geboden.”

Er is geen bewijs dat de Tetracycline de verspreiding van haar kanker gestopt heeft. En artsen adviseren niet dat patiënten zelf gaan experimenteren met dit medicijn, totdat de onderzoeken zijn afgerond. Maar het is een fascinerende nieuwe weg.

“Indien dit medicijn in staat is beschermend en preventief te werken dit soort type metastases, dan is dat een grote opluchting,”zegt Dr. Singh.

A cancer treatment is being developed that's safe, cheap, and almost completely effective. It's been around for decades but no one ever thought about using it to fight the disease. 

The treatment is tetracycline, a family of inexpensive acne antibiotics. The drug is being researched for its ability to prevent metastases, the migration of cancerous tumour cells that migrate throughout the body, creating secondary tumours. 

Dr. Gurmit Singh of the Hamilton Regional Cancer Centre is leading research on the drug. Singh learned over the past four years that tetracycline is not only effective at preventing metastases from forming in bones, it also has the ability to kill tumour cells in the bone.

In tests on mice, his team found the drug cut the spread of certain cancers to the bone by 70 per cent.

"This was very exciting to us so we continued on with our experiments and what we found was that it actually improved the bone itself. In some ways, it healed the bone," says Dr. Singh. 

Tetracycline has been used to treat acne and periodontal disease. The drug is absorbed by teeth and bones, and blocks a group of enzymes called matrix metalloproteinases, a cause of gum degradation. But its efficacy at inhibiting the growth of cancerous osteoclasts in bone has not been fully researched. 

The problem may be the drug's price -- its low price. Because the medication is generic and cheap, there's not much incentive for drug companies to fund research. 

"Pharmaceutical companies, understandably, may not be willing to invest resources in developing research that won't result in a profit for their companies," says Dr. Robert Bell of the Princess Margaret Hospital.

That's what Dr. Singh discovered when he approached some pharmaceutical companies.

"There is no financial gain for industry," says Dr. Singh. "This is a generic drug. If this were a patented drug, then the industry would support it."

The drug has caught the eye of the Canadian Breast Cancer Research Initiative, which is funding a small human study of 60 patients. Dr. Singh is the lead investigator of the project. 

One woman, who wants to be known as Joyce, told CTV's Avis Favaro she heard about the research and decided to try it. Her breast cancer had migrated to her bones. Now she's feeling better.

"I started taking it and noticed differences. I didn't have pain in my back. I got out of the wheelchair. I started walking ... every day I walk at least six blocks."

"It has given me a chance and I think others should be given that opportunity."

There's no proof that the tetracycline had stopped the spread of her cancer. And doctors don't recommend that patients start experimenting on their own, until studies are completed. But it is a fascinating new avenue.

"If this drug is able to protect and prevent those types of metastases, then it is a major relief," says Dr. Singh.
