24 januari 2012: twee grote fase III studies hebben uitgewezen dat als lapatinib - tykerb gegeven wordt naast herceptin - trastuzumab bij vrouwen met Her2 Neu positief de effectiviteit op een complete remissie verdubbelt. Lalapatinib - Tykerb blijkt geen positief effect te geven op ziektevrije tijd, noch op overall overleving bij borstkanker  met Her2-Neu positieve receptoren. Dit meldden onderzoekers op het internationale borstkankercongres in San Antonio. Het is toch ongelooflijk dat dit het zoveelste bewijs is dat een bepaald medicijn dat eerst met grote euforie wordt gebracht in de media enkele jaren later moet erkennen dat het middel eigenlijk gewoon niets toevoegt aan ziektevrije tijd en overall overleving. Medscape schrijft er een heel kort bericht over maar dit bericht is wel helder en duidelijk. Ik heb het artikel zo goed als letterljk vertaald:

GlaxoSmithKline's medicijn Tykerb (lapatinib) faalt in einddoel van een studie bij vrouwen met HER2-Neu positieve borstkanker, daarmee worden hoopvolle verwachtingen de bodem ingeslagen. Engelands grootste farmaceutische bedrijf zegt dat resultaten uit een fase III studie waarin Tykerb gegeven wordt na operatie of chemotherapie geen statistisch significante positieve resultaten laat zien en niet het einddoel heeft bereikt. De resultaten van de zogenoemde TEACH studie werden gepresenteerd op donderdag op het San Antonio Breast Cancer Symposium.

De teleurstellende resultaten bevestigen teleurstellende resultaten uit eerdere studies en analitici van de Deutse Bank zeggen dat de gegevens suggereren dat het niet erg aannemelijk is dat Tykerb - lapatinib een effectief middel kan zijn voor borstkanker. Tykerb, een concurrerend middel met Herceptin (trastuzumab), is al wel goedgekeurd in 107 landen als medicijn voor uitgezaaide borstkanker.

Bovenstaande is dus in tegenspraak met onderstaande informatie al is onderstaand sprake van uitgezaaide borstkanker en bovenstaande van operabele borstkanker stadium I en II.

Lapatinib - Tykerb geeft wanneer gegeven naast herceptin - trastuzumab een significant langere ziektevrije tijd en levensverlenging bij patienten met Her2-Neu positieve borstkanker dan wanneer er alleen herceptin wordt gegeven. Dit blijkt uit een gerandomiseerde fase III studie. Per 1 september 2011 zal lapatinib wprden vergoed voor een kleine groep borstkankerpatienten. Lapatinib is oraal in te nemen en kan ook doordringen in het hoofd om eventueel uitzaaiingen in het hoofd te bestrijden en het geeft veel minder bijwerkingen dan welke chemo dan ook, klik hier voor de op te kunnen treden bijwerkingen Lapatinib wordt dus vergoed voor een bepaalde groep borstkankerpatienten die Her2-Neu positieve tumoren hebben. Maar wordt niet vergoed samen met Xeloda - capecitabine , noch voor hormoongevoelige borstkanker omdat daarvan de meerwaarde niet is bewezen aldus minister Schippers nadat de Telegraaf had gemeld dat dit middel niet vergoed zou worden.

Randomized study of Lapatinib alone or in combination with trastuzumab in women with ErbB2-positive, trastuzumab-refractory metastatic breast cancer.

Abstract

PURPOSE:

Preclinical studies in ErbB2-positive cell lines demonstrated a synergistic interaction between lapatinib and trastuzumab, suggesting that dual blockade is more effective than a single agent alone. EGF104900 compared the activity of lapatinib alone or in combination with trastuzumab in patients with ErbB2-positive, trastuzumab-refractory metastatic breast cancer (MBC).

PATIENTS AND METHODS:

Patients with ErbB2-positive MBC who experienced progression on prior trastuzumab-containing regimens were randomly assigned to receive either lapatinib alone or in combination with trastuzumab. The primary end point was progression-free survival (PFS). Secondary efficacy end points included overall response rate (ORR), clinical benefit rate (CBR; complete response, partial response, and stable disease for >/= 24 weeks), and overall survival (OS).

RESULTS:

In the intent-to-treat population (N = 296) who received a median of three prior trastuzumab-containing regimens, the combination of lapatinib with trastuzumab was superior to lapatinib alone for PFS (hazard ratio = 0.73; 95% CI, 0.57 to 0.93; P = .008) and CBR (24.7% in the combination arm v 12.4% in the monotherapy arm; P = .01). A trend for improved OS in the combination arm was observed (HR = 0.75; 95% CI, 0.53 to 1.07; P = .106). There was no difference in ORR (10.3% in the combination arm v 6.9% in the monotherapy arm; P = .46). The most frequent adverse events were diarrhea, rash, nausea, and fatigue; diarrhea was higher in the combination arm (P = .03). The incidence of symptomatic and asymptomatic cardiac events was low (combination therapy = 2% and 3.4%; monotherapy = 0.7% and 1.4%, respectively).

CONCLUSION:

Despite disease progression on prior trastuzumab-based therapy, lapatinib in combination with trastuzumab significantly improved PFS and CBR versus lapatinib alone, thus offering a chemotherapy-free option with an acceptable safety profile to patients with ErbB2-positive MBC.

Abstract

Aberrant activation of some members of human epidermal growth factor receptor (HER) family plays a key role in breast carcinogenesis. Lapatinib is an oral dual tyrosine kinase inhibitor selective for inhibition of epidermal growth factor receptor (EGFR/ErbB1) and HER2/ErbB2. Having more targets, probably its antitumor activity could be more efficient. Clinical data have shown that lapatinib is active in HER2-positive breast cancer as monotherapy, in combination with trastuzumab, and in trastuzumab-resistant patients. Phase I clinical trials have shown also that lapatinib is well tolerated, with mild diarrhea and skin rush as common toxic effects and low incidence of cardiotoxicity. Phase II and III clinical trials' data provide encouraging evidence of the clinical effectiveness of lapatinib in advanced or metastatic breast cancer and for its potential in patients with brain metastases. Interim results from the large, phase III trial in 392 patients showed that in combination with capecitabine lapatinib almost doubled time to progression when compared with capecitabine alone. Several clinical trials that explore the efficacy of lapatinib in combination with conventional chemotherapeutic agents [paclitaxel (Taxol), capecitabine and platinoids], hormonotherapy and other target therapies are ongoing in advanced breast cancer or in neo-adjuvant and adjuvant settings. Our improved understanding of the biology of breast cancer and the use of biomarkers for identification of specific subtypes are allowing us to bring patient-specific novel therapies such as lapatinib to the clinic.
