Wanneer volwassen gezonde mensen maar met overgewicht van een BMI = Body Mass Index van 25 of hoger op basis van hun vooraf vastgestelde persoonlijke stofwisseling drie maanden lang onder strikte begeleiding een eiwitdieet of een onverzadigdvetdieet gebruikten dan verbeterde hun gezondheid zonder dat iedere deelnemende volwassene direct ook af blijkt te vallen.  Bij alle deelnemers uit beide groepen verminderde het risico op directe hart- en vaatziektes en / of diabetes-2 met 20 procent uit de specifieke dieetgroep en 5 procent uit de andere groep bleek na drie jaar onderzoek. 

Met name de werking van insuline was veel beter als mensen hun persoonlijke dieet hadden gevolgd.


In eerste instantie werden voor de Nederlandse studie (onder supervisie van Ellen Blaak) bij 900 volwassen mensen tussen de 40 en 75 jaar met een BMI = Body Mass Index van 25-40 kg/m2 hun persoonlijke stofwisseling, ook wel fenotype genoemd, gemeten. De wetenschappers keken naar de insuline- en de glucoserespons in de twee uur nadat de deelnemers een suikerdrankje hadden gekregen en op die manier werd hun fenotype, hun stofwisselingsprofiel, gekarakteriseerd. Uiteindelijk bleven 242 mensen over die een gepersonaliseerd dieet kregen om dit drie maanden te gebruiken. Alles  voeding was gebaseerd op de richtlijnen voor gezonde voeding, maar met verschillende verhoudingen in eiwit, vezels en onverzadigd vet.

DIËTEN

AFGESTEMD OP STOFWISSELING

Twee verschillende diëten werden onderzocht, beide op basis van de Schijf van Vijf. De ene helft van de deelnemers in het onderzoek naar stofwisselingstypen kreeg na loting het eiwitdieet en de andere helft het onverzadigd-vetdieet. Voorbeeldmenu’s lieten zien wat deelnemers op een dag moesten eten.

In het eiwitrijke dieet kregen de kandidaten 24 procent van hun energie uit eiwit en 10 procent uit onverzadigd vet.

In het onverzadigd-vetdieet bestond 20 procent van de energie uit onverzadigd vet en 14 procent uit eiwit.

Kwark, yoghurt, 30+-kaas, margarine (voor op brood en om in te bakken), pompoenpitten en een vezelsupplement zaten in het pakket van de eerste groep. De tweede groep kreeg producten als olijf- en zonnebloemolie, tapenade, olijven en kleinere porties yoghurt en kaas.

De deelnemers kregen elke dag een ‘joker’ voor een beetje suiker, snoep of zoet beleg. Ook kregen ze punten voor alcoholische en gesuikerde dranken. Die calorieën waren nodig om voldoende energie te krijgen.

Resultaten:

  • De deelnemers waar eerder een verminderde werking van insuline in de spieren was geconstateeerd hadden duidelijk meer baat bij het hoog-in-eiwit-en-vezels-laag-in-vetdieet.
  • De deelnemers met insulineresistentie in de lever haalden meer gezondheidswinst met het dieet met veel onverzadigde enkelvoudige vetzuren.
  • De insulinegevoeligheid in het hele lichaam verbeterde met 20 procent in de groep die het juiste dieet kreeg, en met 5 procent in de andere groep.
  • Andere parameters, zoals vetten en ontstekingsmarkers in het bloed, bleken ook verbeterd en blijkt risico op diabetes en hart- en vaatziekten te verminderen.

Summary

Precision nutrition based on metabolic phenotype may increase the effectiveness of interventions. In this proof-of-concept study, we investigated the effect of modulating dietary macronutrient composition according to muscle insulin-resistant (MIR) or liver insulin-resistant (LIR) phenotypes on cardiometabolic health. Women and men with MIR or LIR (n = 242, body mass index 25–40 kg/m2, 40–75 years) were randomized to phenotype diet (PhenoDiet) group A or B and followed a 12-week high-monounsaturated fatty acid (HMUFA) diet or low-fat, high-protein, and high-fiber diet (LFHP) (PhenoDiet group A, MIR/HMUFA and LIR/LFHP; PhenoDiet group B, MIR/LFHP and LIR/HMUFA). PhenoDiet group B showed no significant improvements in the primary outcome disposition index, but greater improvements in insulin sensitivityglucose homeostasis, serum triacylglycerol, and C-reactive protein compared with PhenoDiet group A were observed. We demonstrate that modulating macronutrient composition within the dietary guidelines based on tissue-specific insulin resistance (IR) phenotype enhances cardiometabolic health improvements. Clinicaltrials.gov registration: NCT03708419, CCMO registration NL63768.068.17.
