Hoewel patiënten besmet met het coronavirus (COVID-19) voor het grootste deel het meeste last hebben van een longontsteking en daarvoor ook in het ziekenhuis worden opgenomen en meestal ook de reden van overlijden als het fout gaat, blijken een aantal patiënten daarnaast ook last te hebben van spijsverteringsproblemen. Genoemd worden diarree en buikpijn, maar het grootste deel heeft / had weinig eetlust. Dat blijkt uit een deelstudie van een veel groter studierapport (Characteristics of and Important Lessons From the Coronavirus Disease 2019 (COVID-19) Outbreak in China) dat is opgemaakt n.a.v. de uitbraak in China in de provincie Hubei. 

Uit de deelstudie van 206 COVID-19-positieve patiënten in Hubei, China, bleek dat spijsverteringsklachten wel degelijk een rol spelen. En zelfs een slechtere prognose hadden op de overlevingskansen. Zo had 46% (99/206) van de patiënten spijsverteringsklachten: weinig eetlust (83,8%), diarree (29,3%), braken (8,1%) en buikpijn (4%). De diarree werd beschreven als 'los' en tot driemaal per dag.

Degenen met spijsverteringsklachten hadden een langere tijd vanaf het begin van de ziekte tot opname in het ziekenhuis en de patiënten waren er ernstiger aan toe dan patienten met alleen ademhalingsklachten, maar zonder spijsverteringsklachten. Dit kan volgens de onderzoekers te maken hebben met het feit dat de diagnose en behandeling vertraagd was geweest omdat spijsverteringsklachten niet eerder werden beoordeeld.
Het kan volgens de onderzoekers ook verband houden met de darm-longas, wat suggereert dat een darminfectie een ernstigere ademhalingsreactie kan bevorderen door ACE2 in de lever te activeren, dysbiose (ontregeling) van het microbioom te creëren en een sterkere systemische instekingsreactie te bevorderen.

Uit de studie kwamen deze belangrijkste punten naar voren:

Spijsverteringsklachten komen vaker voor dan aanvankelijk werd gedacht en moeten worden opgenomen in het screeningproces.
Diarree is het meest specifieke spijsverteringsklacht, maar is niet ernstig en goed mee om te gaan totdat de ziekte erger wordt.
Patiënten met het coronavirus - COVID-19 met spijsverteringsklachten zijn er erger aan toe als ze in het ziekenhuis terecht komen, hebben een langere ziekenhuisopname en een hoger sterftecijfer.
Een klein percentage van de corona patiënten (3%) vertoonde alleen spijsverteringsklachten.

Advies van de Chinese onderzoekers en artsen betrokken bij patiënten met het coronavirus - COVID-19:

Overweeg bij ziekenhuispatiënten de ontlasting te testen op viraal nucleïnezuur. Eerdere studies hebben aangetoond dat het aanwezig is bij 53,4% van de geïnfecteerde patiënten.

BACKGROUND

Since the outbreak of Corona Virus Disease 2019 (COVID-19) in December 2019, various digestive symptoms have been frequently reported in patients infected with the virus. In this study, we aimed to further investigate the prevalence and outcomes of COVID-19 patients with digestive symptoms.

METHODS

In this descriptive, cross-sectional, multicenter study, we enrolled confirmed patients with COVID-19 who presented to three hospitals from January 18th to February 28th, All patients were confirmed by real-time RT-PCR and were analyzed for clinical characteristics, laboratory data, and treatment. Data were followed up until March 5th, 2020.

RESULTS

In the present study, 204 patients with COVID-19 and full laboratory, imaging, and historical data were analyzed. The average age was 54.9 years (SD +15.4), including 107 men and 97 women. We found that 99 patients (48.5%) presented to the hospital with digestive symptoms as their chief complaint. Patients with digestive symptoms had a significantly longer time from onset to admission than patients without digestive symptoms (9.0 days vs.7.3 days). Patients with digestive symptoms had a variety of anifestations, such as anorexia (83 [83.8%] cases), diarrhea (29 [29.3%] cases), vomiting (8 [0.8%] cases), and abdominal pain (4 [0.4%] cases). In 7 cases there were digestive symptoms but no respiratory symptoms. As the severity of the disease increased, digestive symptoms became more pronounced. Patients without digestive symptoms were more likely to be cured and discharged than patients with digestive symptoms (60% vs. 34.3%). Laboratory data revealed no significant liver injury in this case series.

CONCLUSION

We found that digestive symptoms are common in patients with COVID-19. Moreover, these patients have a longer time from onset to admission and their prognosis is worse than patients without digestive symptoms. Clinicians should recognize that digestive symptoms, such as diarrhea, may be a presenting feature of COVID-19, and that the index of suspicion may need to be raised earlier in at-risk patients presenting with digestive symptoms rather than waiting for respiratory symptoms to emerge. However, further large sample studies are needed to confirm these findings.
